OLA Proposed Amendment Form

The Ontario Lacrosse Association invites Members in Good Standing to submit proposed amendments for consideration at the 2026 Annual General Meeting (AGM).

All proposed amendments must be submitted using the Official Proposed Amendment Submission Template. Before proceeding, please download the template using the link below, complete all required sections, save the document, and upload the completed file later in this form.

Submission Deadline: September 15, 2026. Late submissions will not be considered.

Only proposed amendments submitted using the official template will be considered. Incomplete submissions, or submissions not using the required template, will not be accepted for inclusion.
