By becoming apart of our FYL family, you acknowledge that photos and video recordings may be taken during classes and events. These images/videos may be used for promotional purposes on social media, flyers, websites, and other marketing materials to help showcase our classes, celebrate our community, and inspire others to join our FYL family. Your participation grants permission for your image and likeness to be used without compensation.
Assumption of Risk

I, the undersigned parent/legal guardian, understand that participation in dance classes, rehearsals, performances, competitions, recitals, community events, field trips, workshops, and all other activities sponsored by Find Your Light Multicultural Dance Company involves physical activity and carries inherent risks of injury.

These risks may include, but are not limited to:

Slips, trips, and falls
Muscle strains, sprains, fractures, or other physical injuries
Injuries resulting from dance movements, equipment, or other participants
Transportation-related incidents during approved activities
Weather-related conditions during outdoor events
Any other injuries that may occur while participating in program activities.
I voluntarily allow my child to participate in these activities knowing and accepting these risks.
Release of Liability

In consideration of my child’s participation in Find Your Light Multicultural Dance Company activities, I release, waive, and discharge:

Find Your Light Foundation, Inc.
Its employees, instructors, volunteers, board members, assistants, contractors, event hosts, sponsors, and affiliated facilities
from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising out of or related to my child’s participation in any Find Your Light activity, whether held:

At the dance studio
At rehearsal locations
At community events
During performances or recitals
During competitions
At outdoor venues
At partner facilities
During approved travel associated with Find Your Light events
except in cases involving gross negligence or willful misconduct as provided by applicable law.
