Dragonfly Sand Play Participant Waiver, Release of Liability & Parental Consent
Please read this Agreement carefully before signing.
This Waiver and Release of Liability ("Agreement") is entered into between Dragonfly Sand Play ("Company") and the parent or legal guardian of the child(ren) listed below.
ASSUMPTION OF RISK: I understand that participation at Dragonfly Sand Play includes activities involving sand play, sensory materials, toys, climbing, movement, educational activities, birthday parties, and other recreational activities. I voluntarily accept all risks associated with my child's participation, including but not limited to slips, falls, collisions, minor injuries, allergic reactions, or other unforeseen accidents.
PARENTAL SUPERVISION: I understand that Dragonfly Sand Play is NOT a daycare. I agree that I am solely responsible for supervising my child(ren) at all times while on the premises unless my child is participating in an authorized staff-supervised program.
RELEASE OF LIABILITY: To the fullest extent permitted by Florida law, I release and hold harmless Dragonfly Sand Play, its owners, officers, employees, volunteers, contractors, and agents from any claims, injuries, damages, or losses arising from participation in activities, except in cases of gross negligence or intentional misconduct.
MEDICAL EMERGENCY AUTHORIZATION: In the event of a medical emergency involving my child, I authorize Dragonfly Sand Play staff to contact emergency medical services (911) and seek emergency medical treatment if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred.
HEALTH ACKNOWLEDGMENT: I certify that my child is physically able to participate. I agree not to bring my child if they have fever, vomiting, diarrhea, a contagious illness, or any condition that could place others at risk.
PERSONAL BELONGINGS: Dragonfly Sand Play is not responsible for lost, stolen, or damaged personal belongings.
PHOTO & VIDEO RELEASE: Please choose one: YES, I authorize Dragonfly Sand Play to use photographs or videos of my child for marketing, advertising, social media, and promotional purposes. NO, I do not authorize the use of photographs or videos of my child.
RULES: I agree to follow all facility rules, including parents/guardians supervising their children, shoes being removed in designated play areas, no food or drinks inside the sand areas, respecting staff instructions, respecting other families and children, and prohibiting rough play.
ANNUAL WAIVER: This Agreement shall remain valid for one (1) year from the date signed unless revoked in writing.
GOVERNING LAW: This Agreement shall be governed by the laws of the State of Florida.
ACKNOWLEDGMENT: I certify that I have read this Agreement completely. I understand its contents. I voluntarily agree to all terms.
