Once I have selected a section and costume, I understand that no changes or switches will be allowed. I confirm that all information I provide on this registration form is accurate. I acknowledge that I am fully responsible for the payment of my child’s selected costume as outlined.
FINAL PAYMENTS ARE DUE MONDAY AUGUST 31st, 2026
I give D’Midas Intl NY, its employees, or agents full permission to take photographs, videos, or digital recordings of my child. I understand that these images may be used in any media format, now or in the future, including but not limited to promotional materials, marketing, and social media. I agree that I will not receive any financial or other compensation for the use of my child’s image. I grant D’Midas Intl NY the right to share, post, and distribute this content publicly or privately in both print and digital form.
