OPMF08_Equipment Defect Report

1. Please fill out all required information;
2. Your Name will remain confidential;
3. The Reporter, the National Safety Officer, and Operations will automatically receive a PDF copy of their application.

This form is a controlled document, managed by the NZHGPA, under the delegation of the Civil Aviation Authority (CAA).

1. The Pilot is to report any DEFECTS with paraglider, hang glider, and speedwing equipment in NZ; 
2. Examples of DEFECTS:
   1. incorrectly installed reserves;
   2. carabiners not fit for purpose;
   3. non rated maillons
   4. faulty equipment;
   5. unacceptable repairs;     
3. If your HG/PG equipment was damaged on LAUNCH/LANDING/DURING FLIGHT, fill out the AIRS REPORT;
4. The NZHGPA keeps a database of official safety notices issued by manufacturers for defective equipment.  CLICK HERE to check if the defect you are reporting has had a notice issued;
5. This report will be sent to the NSO and followed up by your Club Safety Officer;
6. Your report will be held on the NZHGPA database and personal details will be held in confidence;
7. There is an option to take photos and upload files (max 5MB).
