• Rest Easy Pet Quality of Life Assessment

Welcome, and thank you for taking the time to complete the Rest Easy Pet Quality of Life Assessment. This veterinarian-designed assessment is designed to help you reflect on your pet’s current wellbeing, monitor changes over time, and identify areas where they may benefit from additional support. It can also provide a helpful starting point for quality-of-life conversations and decision-making if their needs change. There are no right or wrong answers. Please answer each question based on your own observations of your pet over the past month. The assessment takes approximately 15–25 minutes to complete. Your responses will be reviewed by the Rest Easy Pet veterinary team, and you’ll receive a summary of your results at the end. You’ll also have the option to arrange a one-on-one veterinary consultation if you’d like further guidance.
