Our Whole Lives is an age-appropriate, comprehensive, and progressive sex education curriculum that supports participants in making informed and responsible decisions about their sexual health and behavior.
There are two sleepovers scheduled and one evening activity planned for the youth.

For your child to participate, parents must attend a Mandatory Parent Orientation:

I give permission for the above-named minor child (hereinafter "Minor") to be photographed and/or video recorded by Common Flame.

I understand and agree Common Flame may use the photographs and/or video recordings containing the words, image, and/or voice of the Minor in the recorded worship service, special church communications, or functions at Common Flame and may be shared publicly through our website or social media.
