I understand that:
- Photographs and measurements may be maintained as part of my treatment record and used to monitor my progress.
- With my consent, photographs may also be used for educational or promotional purposes, including social media, website content, presentations, or marketing materials.
- Reasonable steps will be taken to protect my privacy and personal information.
- My consent is voluntary, and I may withdraw permission for future use by notifying Bottumzup Health and Wellness in writing.
- I have had the opportunity to ask questions and understand how my photographs and measurements may be used.
