Mission Trip Participant Release, Assumption of Risk, Indemnity & Cancellation
Agreement

I desire to participate in the above-mentioned mission trip sponsored by Suncrest Worship ("Church"). I understand that participation is voluntary and involves travel, ministry activities, transportation, lodging, recreation, and other related activities that may involve certain inherent risks.
By signing this Agreement, I acknowledge that I have carefully read and understand its contents and voluntarily agree to all of its terms.

I understand that participation in a mission trip involves risks that cannot be completely eliminated, including but not limited to:

• International and domestic travel;
• Transportation by air, bus, van, automobile, boat, or other means;
• Illness or injury;
• Food- or water-borne illness;
• Exposure to communicable diseases;
• Weather-related hazards;
• Political instability, civil unrest, crime, terrorism, or natural disasters;
• Delays, cancellations, interruptions of travel, or changes to itinerary;
• Physical activities associated with ministry projects;

• Medical emergencies where care may be limited or delayed.

I voluntarily assume all known and unknown risks associated with my participation.

To the fullest extent permitted by law, I release, waive, discharge, and covenant not to sue Suncrest Worship, its pastors, elders, employees, volunteers, ministry leaders, agents, representatives, and affiliated organizations from any and all claims, liabilities, demands, causes of action, damages, losses, costs, or expenses arising out of or relating to my participation in this mission trip, including personal injury, illness, property damage, disability, or death, except where caused by gross negligence or willful misconduct.

I agree to defend, indemnify, and hold harmless Suncrest Worship, its pastors, employees, volunteers, leaders, and representatives from any claims, liabilities, damages, judgments, costs, or attorney fees arising from my own actions, misconduct, negligence, or failure to follow the instructions and policies established for this mission trip.

I agree to:

• Conduct myself in a manner consistent with biblical values and the standards of Suncrest Worship.
• Follow all instructions provided by trip leaders.
• Respect local laws, customs, and ministry partners.
• Refrain from illegal drug use, alcohol misuse, or other unlawful conduct while participating in the trip.
• Immediately report injuries, illnesses, or emergencies to a trip leader.

I understand that failure to comply with these expectations may result in my removal from ministry activities or being sent home at my own expense if circumstances warrant.

I certify that I am physically able to participate in this mission trip or have disclosed any relevant
medical conditions to the Church.
In the event of illness or injury requiring emergency medical treatment, I authorize Suncrest
Worship and its designated leaders to obtain emergency medical care on my behalf if I am unable
to provide consent.
I understand that I am financially responsible for medical expenses not covered by my personal or
travel insurance.

I understand that mission trips may be delayed, modified, postponed, shortened, or canceled due
to circumstances including, but not limited to:

• Personal emergencies;
• Airline schedule changes;
• Government restrictions;
• Political unrest;
• Natural disasters;
• Public health concerns;
• Safety considerations;
• Ministry partner requests; or
• Other events beyond the control of Suncrest Worship.

If I cancel my participation for any reason, or if the Church cancels or modifies the trip because
of circumstances beyond its reasonable control, I understand and agree that reimbursement will
be limited solely to funds actually recovered by Suncrest Worship from airlines, hotels, ground
transportation providers, ministry partners, or other vendors.
I acknowledge that many travel expenses are non-refundable and that Suncrest Worship does not
guarantee any refund of trip payments. The Church shall have no obligation to reimburse any
amount that cannot be recovered from third-party travel providers or vendors.

I understand that I am responsible for maintaining any personal health insurance and for complying with any travel insurance requirements established by Suncrest Worship. I understand that travel insurance may not reimburse every loss or expense.

I grant Suncrest Worship permission to photograph, video record, and use my likeness for ministry, promotional, educational, and communication purposes without compensation unless I notify the Church otherwise in writing before departure.

This Agreement shall be governed by the laws of the State of Washington. Any legal action relating to this Agreement shall be brought in a court of competent jurisdiction in Washington unless otherwise required by law.

If any provision of this Agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.

I certify that:

• I have carefully read this entire Agreement.
• I understand the risks involved.
• I understand the cancellation and reimbursement policy.
• I understand that I am giving up certain legal rights by signing this Agreement.
• I sign this Agreement voluntarily and without coercion.

I am the parent or legal guardian of the above-named participant. I have read this Agreement, understand its contents, consent to my child's participation, and agree to all of its terms on behalf of myself and my child to the fullest extent permitted by law.

I understand that I am responsible for maintaining any personal health insurance and for complying with any travel insurance requirements established by Suncrest Worship. I understand that travel insurance may not reimburse every loss or expense.

10. Governing Law This Agreement shall be governed by the laws of the State of Washington. Any legal action relating to this Agreement shall be brought in a court of competent jurisdiction in Washington unless otherwise required by law.

11. Severability If any provision of this Agreement is determined to be unenforceable, the remaining provisions

shall remain in full force and effect.

12. Acknowledgment I certify that:

I have carefully read this entire Agreement.

I understand the risks involved.

I understand the cancellation and reimbursement policy. I understand that I am giving up certain legal rights by signing this Agreement. I sign this Agreement voluntarily and without coercion.

Parent/Guardian Consent (Required for Participants Under 18) I am the parent or legal guardian of the above-named participant. I have read this Agreement, understand its contents, consent to my child's participation, and agree to all of its terms on behalf of myself and my child to the fullest extent permitted by law.
