Credit Card Authorization Form

Wildcat Travel Group understands that providing credit or debit card information requires trust. This Credit Card Authorization Form is used to document your permission for Wildcat Travel Group and, when applicable, the travel suppliers involved with your reservation to process approved charges related to your travel arrangements.

Please review this authorization carefully before entering your payment information. By completing and electronically signing this form, you confirm that you understand and agree to the authorization, security, payment, cancellation, and supplier provisions described below.

Completion of this form does not necessarily mean that your card will be charged immediately. Your card will be charged only for the amount, travel service, supplier, and payment purpose identified in this form or otherwise approved by you in writing.

By submitting this form, I certify that:

1. I am the authorized cardholder or an authorized user of the credit or debit card identified in this form.
2. The name, billing address, card information, and other information I provide are accurate and complete.
3. I authorize Wildcat Travel Group to provide the necessary payment information to the applicable travel supplier, tour operator, cruise line, airline, hotel, resort, transportation provider, travel insurance provider, merchant processor, or other authorized service provider for the purpose of completing the travel transaction described in this form.
4. I authorize the applicable merchant or travel supplier to charge the card for the amount identified in this form, subject to the supplier’s payment schedule, terms, conditions, and cancellation policies.
5. I understand that the merchant name appearing on my card statement may be the name of the cruise line, airline, hotel, tour operator, travel protection company, payment processor, or another travel supplier rather than Wildcat Travel Group.
6. I understand that a reservation may result in multiple separate charges from one or more suppliers. These separate charges may equal the total amount I authorized but may appear individually on my card statement.
7. I understand that my authorization applies only to the transaction, amount, payment schedule, and travel arrangements described in this form unless I provide additional written authorization.
8. I understand that Wildcat Travel Group will not intentionally process an amount greater than the amount authorized without obtaining additional authorization from me, except for minor supplier-imposed adjustments that I have expressly approved or that are clearly disclosed in the applicable booking terms.

Wildcat Travel Group takes reasonable administrative, technical, and organizational measures to safeguard the personal and payment information submitted through this form.

When this form is submitted through our approved secure form platform, the information is transmitted through an encrypted connection designed to reduce the risk of unauthorized interception while the information is traveling between your device and the form platform.

Where enabled and supported by our form provider, submitted form information is also stored in an encrypted format. Access to completed forms is limited to authorized individuals who have a legitimate business need to review the information for reservation, payment, accounting, customer service, compliance, or dispute-resolution purposes.

Wildcat Travel Group does not sell payment card information. Payment information is used and disclosed only as reasonably necessary to:

• Process the authorized transaction;
• Complete or manage the requested travel reservation;
• Communicate with the applicable travel supplier or payment processor;
• Address payment questions, refunds, disputes, or charge inquiries;
• Meet accounting, legal, regulatory, or record-retention responsibilities; and
• Protect against suspected fraud, unauthorized transactions, or misuse.

Although reasonable security measures are used, no website, form platform, electronic transmission, or storage system can be guaranteed to be completely secure. Clients should use a trusted device and a secure internet connection when completing this form.

I agree to review my card statement and promptly contact Wildcat Travel Group if I have a question about a charge associated with my reservation.

Wildcat Travel Group will make a reasonable effort to help identify the supplier, reservation, and purpose of an authorized charge. Nothing in this authorization limits any right or remedy available to the cardholder under applicable law, the cardholder’s agreement with the card issuer, or applicable payment-card network rules.

By signing and submitting this form electronically, I consent to the use of electronic records and electronic signatures for this authorization.

I understand that my electronic signature, typed name, date and time of submission, email address, internet protocol information, or other electronic verification information may be maintained as part of the authorization record, subject to the capabilities and configuration of the form platform.

I agree that my electronic signature is intended to have the same effect as a handwritten signature to the extent permitted by applicable law.

I understand that I may request a copy of this completed authorization for my records, provided that any displayed payment card number may be masked or redacted for security purposes.

The Wildcat Travel Group Terms and Conditions contain important information concerning your reservation, including traveler responsibilities, supplier requirements, payment obligations, cancellations, changes, refunds, travel documentation, passport and entry requirements, travel protection, limitations of liability, and other conditions applicable to the travel services you have requested.

By signing and submitting this Credit Card Authorization Form, you acknowledge and certify that you:

• Were provided with access to the Wildcat Travel Group Terms and Conditions before signing this form;
• Had the opportunity to open, read, download, save, and ask questions about the Terms and Conditions;
• Have read, understand, and voluntarily agree to be bound by the Terms and Conditions;
• Understand that the Terms and Conditions are incorporated into and made a part of this Credit Card Authorization Form by reference; and
• Understand that your electronic signature below constitutes your acceptance of both this Credit Card Authorization Form and the Wildcat Travel Group Terms and Conditions.
