Welcome to the CTC Monitoring Program provided by Clean Truck Compliance Solutions, LLC (CTCS).

Thank you for choosing CTCS as your compliance monitoring partner.

Our goal is simple: to make California emissions compliance easier by providing automated monitoring, timely notifications, and professional support so you can spend less time managing compliance and more time running your business.

This Enrollment Agreement summarizes the key terms of your participation in the CTC Monitoring Program. It is intended to provide a clear overview of how the program works and what each of us can expect from the other.

A complete copy of the CTC Monitoring Program Agreement & Terms of Service is available for download and forms part of this enrollment.

By signing below, you acknowledge that you have reviewed this Enrollment Agreement, understand the basic terms of participation, and agree to the current version of the CTC Monitoring Program Agreement & Terms of Service.

Program Overview
The CTC Monitoring Program is designed to assist customers with California emissions compliance by utilizing approved monitoring equipment, automated testing when applicable, and ongoing compliance notifications.

Depending on your vehicle and enrollment, services may include:

• Remote emissions compliance monitoring

• Automated testing when required

• Compliance notifications and reminders

• Equipment installation and support

• Customer assistance regarding monitoring and compliance

CTCS is committed to providing professional service, accurate information, and responsive customer support throughout your participation in the program.

Customer Responsibilities
To help ensure the program operates effectively, customers agree to:

• Provide accurate customer and vehicle information.

• Keep contact and billing information current.

• Maintain their vehicle in safe operating condition.

• Promptly address emissions-related repairs when necessary.

• Respond to reasonable requests from CTCS regarding monitoring or compliance.

• Exercise reasonable care of any monitoring equipment provided by CTCS.

• Notify CTCS if a vehicle is sold, retired, transferred, or undergoes significant emissions-related modifications.

The success of the CTC Monitoring Program depends upon cooperation between CTCS and the customer. While CTCS provides monitoring and support, customers remain responsible for maintaining their vehicles and complying with all applicable laws and regulations.

Monitoring Equipment
If monitoring equipment is provided as part of your enrollment:

• The equipment remains the property of Clean Truck Compliance Solutions, LLC unless otherwise agreed in writing.

• Equipment that becomes defective during normal operation will be repaired or replaced by CTCS at no charge.

• Customers are responsible for exercising reasonable care while the equipment is installed on their vehicle.

• Lost, intentionally damaged, destroyed through misuse, or unreturned equipment may result in a replacement charge of $150.

We understand that accidents happen. If circumstances arise involving your equipment, we simply ask that you communicate with us so we can work together toward a fair solution.

Billing
Enrollment may include:

• One-time setup fee

• Annual monitoring subscription

• Optional installation or mobile service fees

Subscription renewals will be billed according to your selected service plan unless canceled in accordance with the Master Agreement.

Important Information
The CTC Monitoring Program helps simplify emissions compliance.

However, participation in the program does not guarantee that a vehicle will pass any emissions inspection or remain compliant with California regulations.

Customers remain responsible for maintaining properly operating vehicles and completing any repairs required to achieve compliance.
