I, the undersigned parent or legal guardian, understand that participation in the Climb and Maintain ACE Camp (the "Camp") is completely voluntary. I acknowledge that camp activities involve inherent risks of injury, illness, or property damage.

Release of Liability:

In consideration for my child being permitted to participate in the Camp, I hereby agree to release, waive, acquit, and forever discharge KC Metro Avators, its directors, officers, employees, volunteers, agents, and representatives (collectively, the "Released Parties") from any and all liability, claims, demands, or causes of action arising out of ordinary negligence.

This includes any injury, illness (including communicable diseases), death, or property loss suffered by my child while participating in, traveling to, or traveling from any Camp activity. I promise not to initiate any lawsuit or legal proceeding against the Released Parties regarding these matters.

Indemnification Agreement:

I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities—including attorney's fees—brought as a result of my child's involvement in the Camp, or brought by me, my child, or anyone acting on our behalf.

Medical Authorization and Consent to Treatment.

In the event of an emergency, illness, or injury, I hereby authorize the Camp staff to secure any necessary medical treatment for my child. This includes examination, anesthesia, blood transfusion, medication, or surgery by a licensed physician or healthcare provider. I understand that the Camp does not provide medical or accident insurance. I agree to assume full financial responsibility for all medical expenses incurred for my child's care.

Photo and Media Release:

I hereby grant KC Metro Aviators permission to take photographs, video recordings, or digital media of my child during Camp activities. I agree that the organization may use these materials in promotional brochures, websites, social media, or other publications without compensation to me or my child.

Binding Acknowledgment and Signature:

I certify that I am the parent or legal guardian of the camper named above. I have read this entire document, fully understand its terms, and understand that I am giving up substantial legal rights for myself and my child by signing it. I sign this document freely and voluntarily.
