Waiver & Release
1. Assumption of Risk
I understand that participation in archery involves physical activity and the use of bows, arrows, targets, range equipment, and related training activities. I understand that risks may include, but are not limited to, slips, trips, falls, strains, sprains, bruises, accidental contact with equipment, puncture wounds, eye injury, hand or arm injury, back or shoulder strain, or other bodily injury. I voluntarily choose to participate and accept full responsibility for any risk of injury, illness, loss, or damage that may result from my participation.
2. Release of Liability and Hold Harmless Agreement
In consideration of being allowed to participate in this event, I release, waive, discharge, and agree to hold harmless the Muslim Education Center (MEC), Muslim Community Center, FUN ARCHERY, their officers, board members, employees, volunteers, instructors, representatives, agents, sponsors, and facility owners from any and all claims, demands, liabilities, injuries, damages, losses, or expenses arising out of or related to my participation in the archery activity, whether occurring before, during, or after the event on the premises.
3. Safety Rules and Participant Conduct
I agree to follow all safety instructions given by instructors, range safety staff, volunteers, and MEC representatives. I understand that I may not handle bows, arrows, or equipment unless instructed. I agree to point all equipment only in the approved safe direction, remain behind the designated shooting line when required, retrieve arrows only when instructed, and immediately stop all activity when directed. MEC may remove any participant who acts in an unsafe, aggressive, disruptive, or inappropriate manner.
4. Medical Responsibility
I confirm that I am physically able to participate in this activity. I understand that it is my responsibility to consult a medical professional before participating if I have health concerns, medical conditions, prior injuries, or physical limitations. I agree to stop participating immediately if I feel pain, dizziness, discomfort, shortness of breath, or any concerning symptoms. In an emergency, I authorize event staff or volunteers to seek emergency medical assistance on my behalf, and I understand that I am responsible for any medical costs or expenses that may result.
5. Equipment and Personal Property
I understand that equipment may be provided for use during the event and must be used only as instructed. I agree not to misuse, damage, alter, or remove any equipment. I understand that MEC is not responsible for lost, stolen, or damaged personal property.
6. Minor Participants
For participants under 18 years old, a parent or legal guardian must sign below. I am the parent or legal guardian of the minor participant named above. I give permission for the minor to participate in this event and agree to all terms of this waiver on the minor participant’s behalf.
Acknowledgment
I have read and understand this waiver. I understand that by signing this document, I am giving up certain legal rights, including the right to bring claims against FUN ARCHERY USA, MEC, MCC, event organizers, volunteers, instructors, or representatives for injuries or damages related to this event.
