    Proof of entitlement must be current, a letter, dated no more than three months before date of enrolment. The JSA booklet is NOT acceptable proof. For Universal Credit we will accept a print-out from the website benefit account.

  • Why do you need to provide evidence?

    The Greater London Authority / Mayor of London funds our courses, and condition of this funding is to supply learner data that demonstrates that we are using the public fund correctly. We also need to know who our learners are so we can plan our programme to suit the needs of each individual.

    General Data Protection Regulation (GDPR) 2018 - CONSENT TO PROCESS

    PDT collects information about all our staff and learners for various administrative, academic and health and safety reasons. Due to the GDPR and the Data Protection Act 1998, we need your consent before we can do this. We cannot operate the service effectively without processing information about you, we need you to sign the consent to process clause. If you do not do so, we will be unable to offer you a place and may withdraw any offer already made. If you require any further information about this, please contact our team.

    The information will be used for the following purposes:

    Transmission of appropriate learner records information for external bodies, Awarding Bodies and administrative purposes. Managing and maintaining a safe environment.

    Monitoring and improving the quality of provision. Monitoring for equal opportunities purposes.

    I agree to PDT collecting, recording and processing my personal data as contained in this form, or other data or images which PDT may obtain from me or other relevant agencies, whilst I am a learner. I agree that the provider will process such data only for purposes connected to my studies or my health and safety whilst on the premises or for other statutory purposes.

    The information to be recorded and processed (at the date of this agreement) has been completed by me and I confirm that it is correct. I also note that you may need to process sensitive personal data on me in respect of any special needs I may have, to enable you to deliver the agreed course properly and to have proper regard for my health, safety and well-being whilst I am a learner with you. This will enable you to fulfil your obligations to me under this Learner Agreement and as required by law, and I consent to such use.

  • Learner Declaration and Commitment;

    I declare that all information provided by me in this enrolment form is true, accurate and complete to the best of my knowledge. I understand that PDT reserves the right to withdraw or cancel my enrolment if any information I have supplied is found to be false or misleading.

    I understand that the Provider may access my Personal Learning Record (PLR) through the Learning Records Service (LRS) for the purposes of confirming my prior qualifications, supporting funding eligibility checks, preventing duplicate funding, and ensuring the accuracy of information recorded about my learning.

    I consent to the use of my personal data for all purposes connected with my learning, participation, and health and safety while on the provider’s premises or engaging in programme activities. This includes any contractual or administrative requirements, and specifically the disclosure and processing of all relevant data in accordance with the Greater London Authority (GLA) Privacy Notice, outlined in Annex 5.

    I confirm that I have received Initial Assessment and Information, Advice and Guidance (IAG) relating to my chosen course. This included clear information about the course content, entry requirements, the implications of choosing this programme, its suitability for my learning and career goals, and the support available to me throughout my studies.
