This form provides parental/guardian consent for a student's participation in church-sponsored on-site and off-site events and ministries and ensures that parents/guardians understand and acknowledge the potential risks associated with participation and transportation.
Consent to Participate
I, the undersigned parent or legal guardian, hereby give permission for my child to participate in the on-site and off-site events and ministries organized by Bethel Youth (Bethel Church), including all scheduled activities and transportation to and from the event.
Activities may include, but are not limited to: local travel plans, youth conferences/concerts/retreats, bowling, corn maze trips, ice skating, games in the park, soccer, volleyball, softball, baseball, basketball, dodgeball, hiking, Bible studies, small groups and off-site day events.
Any overnight events and/or higher-risk activities (such as rock climbing, swimming, and river floating) will require a separate registration and consent form prior to participation.
Transportation may include personal vehicles operated by approved Bethel Church leaders, volunteers, or parents/guardians; rented vehicles; chartered buses; ferries; public transportation; or other transportation arrangements organized by Bethel Church.
Parents/guardians may withdraw permission for their child to participate in a specific activity by providing written notice to the Family Pastor. Withdrawal of permission will take effect once the church has received and acknowledged the notice.
Acknowledgement of Potential Risks
I understand that participation in on-site and off-site events may involve risks, including but not limited to:
General Risks
- Trips, slips, and falls
- Minor injuries such as sprains, cuts, bruises
- Contact with environmental hazards (uneven ground, weather, natural surroundings)
- Loss or damage of personal belongings
Transportation-Related Risks
- Motor vehicle collisions or breakdowns
- Delays or unexpected route changes
- Injuries while entering/exiting vehicles
Activity-Specific Risks
- Sports-related injuries
- Water activities (drowning, slips on wet surfaces)
- Hiking or outdoor exploration (insect bites, allergic reactions, rough terrain)
- Use of equipment or tools (sports equipment, games, etc.)
- Seperation from the group or becoming lost
Health-Related Risks
- Allergic reactions
- Illness during the event
- Exposure to contagious conditions
- Medical emergencies requiring first aid or medical support
- Sun exposure, dehydration, or weather-related illness
Interpersonal or Behaviour-Related Risks
- Accidental injury caused by another participant
- Behavioural issues requiring early pick-up
- Emotional stress or conflict with peers
Unforeseen Risks
I acknowledge that risks may exist that are not reasonably foreseeable or listed above.
Medical Consent
I authorize adult leaders of Bethel Church to:
- Provide first aid or basic medical care
- Seek emergency medical treatment if deemed necessary
- Transport or arrange transportation for my child to a medical facility
If an emergency arises, Parent and Guardians will be contacted first followed by emergency contacts. I understand that emergency medical treatment will not be delayed while waiting for a parent/guardian to be reached.
I assume all responsibility for medical costs that may result.
Release of Liability
In consideration for allowing my child to participate, I hereby:
- Release and forever discharge Bethel Church, its pastors, employees, volunteers, representatives, and event leaders from any and all liability for injury, illness, accident, property damage, or death that may occur during or as a result of events, except where prohibited by law.
- Agree to indemnify and hold harmless Bethel Church from any claims or demands arising from my child’s participation, whether caused by negligence or otherwise, to the extent permitted by law.
Illness
Bethel Church reserves the right to require a student to remain home or be picked up from an activity if they are experiencing symptoms of illness or another condition that may reasonably pose a health or safety concern to themselves or others.
Behaviour Expectations
I understand that my child must follow all church rules, event guidelines, and instructions from leaders. This includes treating other participants, leaders, volunteers, facilities, and property with respect. Bullying, harassment, fighting, threats, inappropriate sexual behaviour, possession or use of alcohol, cannabis, illegal drugs, tobacco or vaping products, weapons, or other prohibited items will not be permitted. I agree that if behavioural issues arise, my child may be temporarily removed from activities or sent home at my expense.
Notice of Change in Information
I agree to notify Bethel Church of any changes to my child's medical, emergency contact, or other information that may affect their participation or safety.
