I, the undersigned parent or legal guardian of the student named below, voluntarily grant permission for my child to participate in Eagles Club activities sponsored by Providence Christian School ("PCS").
I understand that participation in Eagles Club may involve physical activity, recreational games, outdoor activities, field trips, and other events that carry inherent risks of injury, illness, property damage, or other unforeseen circumstances. I acknowledge that these risks cannot be completely eliminated.
In consideration for allowing my child to participate, I agree to the following:
Assumption of Risk
I understand and voluntarily assume all risks associated with my child's participation in Eagles Club activities, whether known or unknown.
Release of Liability
To the fullest extent permitted by law, I release, waive, and discharge Providence Christian School, its Board of Directors, employees, administrators, faculty, staff, volunteers, agents, and representatives from any and all claims, demands, causes of action, damages, losses, or liabilities arising out of or related to my child's participation in Eagles Club, except to the extent caused by willful misconduct or gross negligence.
Medical Authorization
In the event of an emergency, I authorize Providence Christian School personnel to obtain emergency medical treatment for my child if I cannot be reached promptly. I understand that I am financially responsible for any medical expenses incurred.
Rules and Conduct
I understand that my child is expected to follow all school rules, Eagles Club guidelines, and instructions provided by staff and volunteers. Failure to do so may result in removal from the program without refund.
