Hi Leaders!

We are so excited to have you join the Servant Leader Team for 26-27! This is an incredible opportunity to continue to grow deeper in your own faith, strengthen friendships with peers who share your passion for Christ, and journey with younger teens who are living their own journey towards Confirmation. Our Servant Leaders don't need to have all the answers, they just need to love Christ and the Catholic faith and be ready to be present to the teens in front of them.

Please note that by completing this form you are committing to attend the Servant Leader Retreat, serve through the whole year Aug-May with either Year 1, Year 2, or both Years of Confirmation Preparation AND committing to attending monthly Gatherings/special events consistently.

I know that many of you are involved in other activities as well, so please review the Servant Leader Save the Dates and Confirmation Year 1 & 2 Calendars (linked below) to ensure that you understand the time commitment you are making. An additional commitment form will be distributed on the Servant Leader Retreat to ensure that you understand the expectations of being a role model through words and actions.

Permission, Liability, Media & Medical Waivers
I, [the parent (guardian)] of, name of student(s) hereby give my permission for my or my child's participation in Servant Leader On-Site Gatherings and Confirmation Sessions 2026-2027, Teen Leader Summit on Sunday, August 30, 2026 at Diocese of Orange, and Servant Leader Retreat on Saturday, September 19-20, 2026 at Lazy W Ranch. As a condition of my child(ren) being allowed to do so, I hereby release and discharge the Diocese of Orange, Holy Trinity Catholic Church, its constituent organizations, including, but not limited to, The Roman Catholic Bishop of Orange, a Corporation Sole, and their officers, employees and volunteers from any and all claims for personal injuries or property damage that s(he) may suffer as a result of my/his/her participation in the activity described above, whether or not such injuries or damages are caused by the negligence, active or passive, of any of the entities or individuals named or described above.

I agree to direct my child to cooperate and follow directions and instructions of parish, or Diocesan personnel responsible for all activities.

I agree, that in the event of injury as a result in my or my child(ren)'s participation in the above activity, including transportation to and from these activities, whether or not caused by the negligence, active or passive, of the parish, school, or Diocesan youth activities program or any of its agents, employees or volunteers, recourse for the payment of any resulting hospital, medical, dental treatment or related costs and expenses will first be had against any accident, hospital, medical or dental insurance, or any available benefit plan of mine or my spouse. I am not aware of any medical condition which would render it inappropriate for participation in any activity.

I hereby give permission to the physician, nurse, dentist or licensed care staff selected by the supervisory personnel then present to render medical, dental or other appropriate treatment deemed necessary and appropriate by the physician, nurse, dentist or licensed care staff.

I, hereby, authorize the making of photographs, motion pictures, video tapes, or other recordings memorializing said event and my or my child(ren)’s participation therein, and the publication and duplication or other use thereof. I, hereby, waive any rights to compensation or any right that I otherwise might have to limit or to control such making or use.
BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

Your registration helps cover a portion of Servant Leader Retreat expenses, SL Gatherings meals/snacks, and leadership materials throughout the year. Holy Trinity is honored to cover the remaining cost as an investment in our teen leaders and their growth.
