Before your appointment, please confirm your visit details, add any optional services, and provide any health or care updates. If this information has not been submitted prior to arrival, it will be completed during the drop-off appointment.
  • First Day of Visit
     - -
  • I understand that The Blue Hound operates by appointment only. My scheduled drop-off and pick-up times are appointments, not an open arrival window during business hours. If I need to adjust my appointment time, I must contact The Blue Hound in advance and receive confirmation. Unapproved early or late arrivals may result in additional fees, delays, or an inability to accommodate the requested change.*
  • Add-On Services*
  • Has your dog visited any other grooming salon, boarding facility, or daycare since their last visit?*
  • Has your dog experienced any of the following symptoms within the last five days prior to boarding?*
  • Are there any new health concerns for your dog (diagnosed or not)?*
  • Are there any new medications your dog will require during this stay?*
  • Have there been any changes to care or feeding instructions staff need to know?*
  • I confirm all my dog(s) rabies vaccinations are up to date. If not, I understand vaccinations must be updated prior to boarding to avoid cancellation.*
