Delta Gamma Lambda Foundation Tennis Experience

Dear Parents/Guardians:
The Delta Gamma Lambda Foundations Inaugural Tennis Experience will be held on Sunday, September 20th, at Four Bridges Country Club 8300 Four Bridges Dr. Liberty Township 45044. The participants will receive some general information around the game of tennis in addition to various techniques, and practice drills geared towards improving their skill level and interest in the game of tennis. The event is facilitated by the brothers of Alpha Phi Alpha Fraternity Inc. in conjunction with the tennis coaches, and professionals located at the country club. The Tennis Experience will run from 11:00 AM to 3:00 PM.
Due to the athletic nature of this event all participants 18 and under will need the consent of their parents or guardians in order to participate. If you are over 18 years old please proceed to the participant information to continue completing the registration.
Parent/Legal Guardian (required if participant is a minor): My signature below indicates that I have read, fully understand and accepted all aspects of this document. I hereby consent to first aid, emergency medical care and, if necessary, admission to an accredited hospital for executing such care, for treatment of injuries that my Minor child may sustain while participating in the DGL Tennis Experience. I understand that I will be financially responsible for any and all medical related expenses incurred in treating my child for treatment of aforementioned injuries sustained while participating in this activity.
