Before You Complete This Form

Who should complete this form?

This form is for families whose children attend non-Catholic schools.

If your child attends a Catholic school, please contact the Children's Faith Formation Office, blsacfaithformation@gmail.com, to request the Catholic School Sacrament Preparation Enrollment Form.

Active Member Verification

Verification of Active Member status is required before registering a child for Sacramental Preparation and must be maintained throughout the preparation process.

To become an Active Member, you must:

Be registered as a parishioner at Blessed Sacrament Parish.
Regularly participate in Sunday or Saturday Vigil Mass.
Use your parish offertory envelopes for at least three consecutive months.*

To maintain your Active Member status, offertory envelopes must continue to be placed in the collection, even if no donation is enclosed.

Which Children Should Be Listed?

Please list only children who will be in a Sacramental Preparation year, including:

First Communion – Year 1 or Year 2
Confirmation – Year 1 or Year 2
Baptism Preparation

These are typically students in 1st and 2nd grades (First Communion) or 7th and 8th grades (Confirmation), although some children may prepare at a different age/grade.

Do not list children who are not in a Sacramental Preparation year. You will list all of your children later when you fill out the Faith Formation Registration Form.

If You Are Transferring from Another Parish

If you were an Active Member at your previous parish:

Obtain a letter from your previous parish verifying your Active Member status.
Register as a parishioner at Blessed Sacrament Parish at blsac.org.
Attach your previous parish verification letter when submitting this form.

If you were NOT an Active Member at your previous parish:

Register as a parishioner at blsac.org.
Review Blessed Sacrament's Active Member requirements.
Begin using your parish offertory envelopes at weekend Masses to establish Active Member status.
Wait three weeks to submit this form*

*After registering as a parishioner, please wait three weeks before submitting this form. Your verification letter will indicate a Pending status, which allows you to register your child for Sacramental Preparation. During this time, you are expected to fulfill the parish's Active Member requirements. Families who do not establish and maintain Active Member status may not be eligible to continue in the Sacramental Preparation program.

I understand that my family's status will be reviewed periodically while in Sacramental Preparation. If at a periodic review our status is changed from Active Member Status to Non-Active Member Status, due to not meeting the active criteria set, my child will not be able to continue in the Sacramental Preparation program or receive a Sacrament that year.

Please Note:

If you give online, please turn in your empty envelopes weekly at Mass and check "I give online."
If you contribute periodically through the collection, please remember to put in envelopes weekly, even if empty, and mark "I attended Mass."
Contributions in cash can only be considered when presented in a parish envelope.
Multiple envelopes put in at a weekend Mass will be considered participation for that one Mass.
Envelopes dropped off or mailed will not be considered participation at Mass.
Blank green envelopes are available in the church if needed. Remember to write your full name and address.
