Before completing this form, please read the updated Studio814 Parent Handbook 2026–2027, as you will be asked to sign an agreement at the end.

Lessons start on Tuesday, August 18, 2026. Families may complete this form at any time throughout the 2026–2027 school year.

All required fields must be completed in order to submit the form. Once submitted, please allow us some time to coordinate lesson schedules.
