AFTER SCHOOL @ BRIDGES (A CFCI Funded Program)

Bridges Community Economic Development Corporation (Bridges CEDC) offers a research-based fully sponsored high-quality afterschool program for students in grades 1 through 12. Rooted in a education excellence and the whole-child approach, the program supports students' academic, social, and emotional development in a safe and nurturing environment. Trained and caring staff, including credentialed teachers, are dedicated to meeting each student’s individual needs. We walk alongside you during the school year to help your student flourish in school and beyond.

I,Print Parent/ Guardian Name* hereby give permission for Student Name, a minor child, to participate in the Afterschool @ Bridges. I understand that the primary objective of Bridges CEDC is to support and encourage students to excel academically, improve their critical thinking, advocate for themselves, and receive social-emotional support in a safe, supportive, and nurturing environment.
Parent/Guardian Authorization and Consent
I hereby authorize Bridges CEDC directors, staff, and volunteers to take the following actions:

1. Access to Academic Records
To access, obtain, make, and receive copies of my child’s academic school records while my child is actively enrolled in one or more Bridges CEDC programs. I understand that these records will be kept strictly confidential and used solely to:

a) monitor my child’s academic progress; and
b) determine the academic support services needed to help my child progress toward graduation.

2. Media and Materials Release
To reproduce, publish, and use original materials submitted by program participants, as well as photographs, videos, or other images of participants. I understand that participants’ work or likenesses may be reproduced, in whole or in part, for purposes related to ongoing program promotion and evaluation.

I release Bridges CEDC from any obligation to compensate me, my child, or any person or party acting on my behalf for the authorized use of the materials or media described above.
Medical Treatment Authorization

I, Parent/ Guardian Name* , give my consent for my child to participate in activities offered by Bridges CEDC. In the event of an accident, illness, or medical emergency occurring on or away from the program site, I authorize Bridges CEDC staff to obtain necessary medical treatment for my child, including treatment at the nearest appropriate medical facility.

I understand that I am financially responsible for any costs incurred for such medical treatment, regardless of insurance coverage.

Bridges CEDC Student Policies and Expectations

Read thoroughly with your Student
To maintain a safe, respectful, and productive learning environment, all students participating in Bridges CEDC programs are expected to follow the policies and expectations below.
1. Attendance and Punctuality: Students are expected to arrive on time for all scheduled academic sessions, workshops, presentations, and other activities for which they are registered.
2. Respectful Conduct: Students are expected to treat teachers, tutors, workshop facilitators, staff, volunteers, and fellow students with respect at all times.
3. Appropriate Attire: Students are expected to wear appropriate attire while participating in Bridges CEDC programs and activities. Clothing, attire, or accessories that create a safety concern or substantially disrupt the learning environment are not permitted.
4. Computer and Technology Use: Computers and other Bridges CEDC technology are intended for academic and program-related purposes only. Playing games or downloading unauthorized pictures, music, applications, or other content is not permitted.
5. Participation and Attendance: Students must attend at least once per week to remain in good standing. Two consecutive weeks of absence without prior notification may result in dismissal from the program. Consistent attendance is important to students' academic progress and helps Bridges CEDC meet program and funding requirements.
6. Required Documentation: All required forms and documents must be completed, signed, and submitted in a timely manner.
Prohibited Conduct

The following behaviors are not permitted:
- Disruptive behavior, including playing or excessive talking during academic sessions
- Profanity or inappropriate language
- Hitting, fighting, or other physical aggression
- Bullying, harassment, intimidation, or threatening behavior
- Unauthorized use of cell phones or other electronic devices during classroom sessions
- Possession, use, or distribution of drugs or alcoholic beverages
- Any behavior that creates a safety concern, substantially disrupts the learning environment, or interferes with the well-being or learning of others
Failure to Meet Expectations

Failure to follow these policies and expectations may result in appropriate corrective action based on the nature and severity of the behavior. Actions may include a conference with the student, parent/guardian, and Bridges CEDC staff. Repeated or serious violations may result in suspension or dismissal from the program.

Bridges CEDC is committed to maintaining a safe, respectful, supportive, and productive environment in which all students have the opportunity to learn and succeed.
I acknowledge that I have read and understand the Bridges CEDC Student Policies and Expectations. I understand that students participating in Bridges CEDC programs are expected to follow these policies and that failure to do so may result in corrective action, including suspension or dismissal from the program.
