Once submitted, this form will go to OOAB Secretary and OOAB Treasurer for review and they will reach out to you with Approval or feedback.

If approval is received, the Team is expected to collect money using one of the following processes:
1. Jotform for online payments – request from OOAB Communications
2. PayPal Code – request from OOAB Treasurer
3. Square Code / Square Reader – request code from OOAB Treasurer (team will need their own Square Reader)
4. Cash – collected and deposited by team liaison (deposit books provided by OOAB Treasurer or use this LINK to deposit funds)
