Special requests for more than 1 support person are made on discretion
A non refundable deposit of $50 is required for appointments of 8 or less piercing points
A non refundable deposit of $100 is required for appointments of more than 8 piercing points
Deposit amount will be taken off final total cost on the day of appointment
Cancellation notice is required 48 hours prior to appointment. If cancellation occurs within 48 hours of appointment, deposit will be forfeited
The right of refusal at any time remains with the Artist. If service refusal has occurred, deposits will be forfeited
Submitting this form provides consent for the Artist to contact you to book an appointment
Booking day/time requests will be taken into consideration however does not guarantee availability
