Participation Agreement and Acknowledgment of Risk
I give permission for my child to participate in dance classes and related activities offered by Level Up Dance Complex LLC. These activities may include dance instruction, stretching, conditioning, acrobatics, rehearsals, performances, and other physical movement.

I understand that participation in physical activities involves inherent risks, including slips, falls, collisions, muscle strains, sprains, and other injuries. I confirm that I have disclosed any relevant medical conditions, injuries, allergies, or accommodations that may affect my child’s participation.

I understand that dancers are expected to follow instructor directions, studio rules, and safety requirements. I agree to notify Level Up Dance Complex if my child’s health or ability to participate changes.

To the fullest extent permitted by law, I accept the ordinary risks associated with participation and agree not to hold Level Up Dance Complex LLC, its owners, instructors, employees, contractors, or representatives responsible for injuries or losses resulting from those inherent risks, except where responsibility cannot legally be waived.

Photo and Video Permission
Level Up Dance Complex may photograph or record classes, rehearsals, performances, events, and studio activities for educational, promotional, social media, website, and advertising purposes. Dancers will not be identified by full name without additional permission.
