Personal Safety Lesson
Every year we are required by the archdiocese to teach Personal Safety Lessons (PSL) to all children who are enrolled in Faith Formation. Please reach out to Lisa Sloboden with any questions you have about the Lessons.

Parents may opt-out their child from the Personal Safety Lesson (PSL). However, materials will be sent home for parents to teach them to their children. Please let Lisa Sloboden know if you would like the form to opt out.

By signing this form, I give permission for the children registered on page 1 of this form, to attend the Personal Safety Lesson (PSL) during the Faith Formation program at Most Holy Redeemer Church. Specific dates of each PSL are on the calendar. Reminders of your Child(ren)'s PSL will also be emailed to you before the Lesson date.

Rules for Faith Formation
1. Absence: Students are expected to attend every class. Mass and other events as noted in the calendar count as class time. Please email Lisa Sloboden at Lisa@mosthr.org if your student is unable to attend class.

2. Distractions: The following items are NOT allowed in class. If brought to class they will be placed in a basket and returned after class ends.

Cell phone or any other electronic device
Food
Beverages (soda, juice, etc.) A water bottle is allowed.
Gum
Candy
Toys of any kind

3. Promptness: Students are expected to be in the classroom by 6:30 pm. Please remind your child to use the bathroom at home or right away at school before class begins. Students are finished with class at 7:30 pm. They may not leave the church or school building during Faith Formation without prior approval from their parent/guardian.

4. Respect: Students should respect themselves and others. This includes teachers, helpers, and other students. Students should also respect the school and church buildings, the contents of the desks in which they sit, and the surrounding property.

5. Discipline: If your child displays inappropriate behavior while they are on church or school property, the following will apply:
1. The teacher will verbally correct the student by directing the student to stop the behavior.
2. If the behavior continues, the student will be sent to the Faith Formation Coordinator for the remainder of the class. Contact will be made with the parents/guardians and Father will be notified.
3. If a student is removed from class again, a conference with the student, parents, the Faith Formation leader, and Father will be required before the student is allowed to come back to class.
