UNDER 28 U.S.C. §1746 and the pentalies of perjury under the laws of the United States, I swear that the foregoing is true and correct and that each of the identification documents which I have provided and of which I have signed the attached copy was lawfully obtained by me and has not been forged or altered. Model understands that all the information given in this Record-Keeping Compliance Form is being provided to comply with federal and state laws. I have read this model release form carefully and fully understand its meanings and implications. By submitting this form I agree to all terms and permissions written and verbal from Alexa Taylor.*

2257 Model Release

By signing this release I hereby give the Photographer / Filmmaker and Assigns my permission to license the Content and to use the Content in any Media for any purpose, which may include, among others, advertising, promotion, marketing and packaging. I agree that the Content may be combined with other images, text, graphics, film, audio, audio­visual works; and may be cropped, altered or modified.

I acknowledge and agree that I have no further right to additional consideration or accounting, and that I will make no further claim for any reason to Photographer / Filmmaker and / or Assigns. I acknowledge and agree that this release is binding upon my heirs and assigns. I agree that this release is irrevocable, worldwide and perpetual, and will be governed by the laws (excluding the law of conflicts) of the country/state from the following list that is nearest to the address of the Model given.

I agree that I am receiving no additional compensation, and have no further rights to the content created beyond what is outlined in this agreement.
It is agreed that neither model, photographer, filmmaker, producers, assigns or any other person associated with this production shall have their personal information made publicly available, and the information contained in this release may only be used directly in relation to licensing of the Content where necessary (e.g. to defend claims, protect rights or notify trade unions) and information contained in this release may be retained as long as necessary to fulfill this purpose, including by being shared with sub­licensees / assignees of the Photographer / Filmmaker and transferred to countries with differing data protection and privacy laws where it may be stored, accessed and used. I agree by signing this document will not share the names, personal information, or other sensitive documents I have or am receiving in the course of making the Content with the public, and agree to never post any type of identifying or sensitive information (which includes names, addresses, phone numbers, health information, marital status, ID photos) about anyone appearing in this content, other models, the filmmaker, assigns, or their associates present on set to the general public, including on social media or other platforms where it may be easily, freely, and openly accessed. By signing this agreement, I acknowledge that I agree to keep other’s information confidential.

I certify that I am not under the influence of any drugs or alcohol, and understand that the use of these substances may result in the non-completion of a scene or being asked to leave.

I know and understand that a shortened portion may appear on affiliated sites, social media platforms, as a form of revenue and ad service for the creators of this video content. If content is used this way, filmmaker/assigns will credit the model who appears in this film by model name, link, or other verifiable source. Model/s/performers will reserve the right to have their content removed by asking the poster on any tube site, social media site or other “Free” or open platform by contacting filmmaker directly.

The filmmakers may utilize any and all appropriate platforms to post including platforms that will exist in the future and their subsidiaries for distribution for all time not listed in this release. Additionally promotional content with the signer's image may be posted and promoted on social media and other platforms, including use on future platforms not yet in existence for all time. By signing this release you understand that content may be posted or licensed for distribution to multiple clip and video streaming sites, which may not be named in the release, as well as portions of it used for promotional purposes, on any site the filmmaker chooses. By signing this release model acknowledges and understands that this clip will appear on multiple adult platforms across the internet and may be licensed for use with all listed and other non-listed media sites. Signer understands and agrees that the content is adult in nature and will be published on adult oriented sites.
I the model and signer certify my understanding that I have been appropriately tested to an industry standard that my partner in this scene has verified,or will be taking barrier precautions in this scene, and I certify that I have viewed my partner/s results.

I acknowledge that by signing this document, I certify that I am over 18 years old, as represented by a government issued ID, at the time of this production, pursuant to federal law, which I have provided for examination by the filmmaker on set at the time of filming.
