STUDENT/PARENT/GUARDIAN ATHLETIC PARTICIPATION CONTRACT AND PARENT PERMISSION FORM
The mission of the Winston Churchill High School Ice Hockey Club, Inc. (WCHSHC or "the Club") is to enhance the educational experience of students at Winston Churchill High School by providing them with the opportunity to participate in a competitive high school hockey program.
Through the volunteer efforts of parents and others, the Club strives to provide players with a positive, productive, and rewarding hockey experience.
The privilege of playing on a high school hockey team comes with the responsibility to pursue excellence. Players are expected to do their absolute best, both athletically and academically. They represent the Club, the team, Winston Churchill High School, the Montgomery Hockey Conference (MHC), and the Maryland Student Hockey League (MSHL), both on and off the ice. As representatives of these organizations, players are expected to conduct themselves in a positive, respectful, and disciplined manner.
While fielding competitive teams is an important goal of the Club, it is not the only one. Developing essential life skills—including accountability, cooperation, dependability, discipline, leadership, resilience, responsibility, self-advocacy, and self-esteem—is equally important. A strong foundation in these areas can help athletes thrive in academics, careers, and their personal lives.
Membership in the Winston Churchill Hockey Club should be viewed as an opportunity for young people to build character, learn to work together as one team, and develop socially, emotionally, and physically.
Parents are encouraged to keep these goals in mind when communicating with their children, other Club players, and players and parents from other teams. Through their words and actions, parents play an important role in fostering the positive, respectful, and supportive environment the Club seeks to create.

Parents should remember the job of the coaching staff is to instruct and teach the players. Parents are respectfully asked to avoid giving their player specific instructions regarding techniques, positioning, tactics, strategies, etc. that may conflict with the Club’s coaching philosophy. There are many successful coaching models and if your instructions are different than the team's model, this likely will result in confusion or frustration for your player and undermine the effectiveness of the coaching staff or team as a whole.
Parents and players are asked to review the following information from both the WCHSHC and the Montgomery County Public Schools (MCPS) system that applies to participation in the Club's hockey program.
GENERAL INFORMATION
Philosophy: The philosophy of the Montgomery County Public Schools (MCPS) athletics program is founded in their vision, mission, purpose and R.A.I.S.E. core values:

An important goal of the athletics program is to teach and reinforce values relating to sportsmanship, competition, and fair play. It is expected that team personnel, parents/guardians, and spectators promote the R.A.I.S.E. core values of respect, sportsmanship and spirited competition and those of USA Hockey (https://www.usahockeyrulebook.com/page/7611/vi-codes-of-conduct) by exhibiting appropriate behavior at all Club hockey events.
A. Student Eligibility Requirements
Students must meet requirements, as established by the Maryland Student Hockey League, USA Hockey and the Club to be eligible to participate. Participation of ineligible students will result in individual and team sanctions, including forfeits for the team so it’s important that all players and parents are aware and adhere to these.
1. Residency
Players must live in the Winston Churchill High School cluster. A student’s legal residence must be within the geographic boundary outlined for WCHS as determined by MCPS.
Students whose legal residence is outside the designated boundary of WSHC may NOT participate in the Club unless they have received a Change of School Assignment (COSA) and the WCHSHC has received approval from the Maryland Student Hockey League (MSHL.) The MSHL follows the Montgomery County Board of Education Policy JEE, Student Transfers, and MCPS Regulation JEE-RA, Transfer of Students which require that students attend specific schools within the established attendance area in which they reside, or are assigned in accordance with an Individualized Education Program (IEP).
A COSA is also required when a student moves out of a school’s boundary area during the season and desires to continue to participate on the hockey team. The WCHS Hockey Club must be notified that the player will be moving out of the school's boundary area before the actual move, and the player can not participate in games or practices after the move until the WCHS Hockey Club receives the COSA, submits it to the MSHL, and sends a written approval back to the player authorizing him/her to continue to practice and play.
2. Maximum Age and Years of High School
Students must be younger than 19 years old as of August 31 of the current school year are ineligible, and are limited to eight consecutive semesters of eligibility starting from their freshman year.
3. Pre-participation Physical
Students must submit a current MCPS Pre-participation Physical Evaluation Form (MCPS Form SR-8), and a Student/Parent/Guardian Athletics Participation Contract before being allowed to participate in practices or contests.
4. USA Hockey Registration and SafeSport Training
All players must be registered with USA Hockey; the fee for this registration must be paid before tryouts begin to ensure players are covered by insurance. USA Hockey requires players who turn 17 years of age or older on or before December 31 of the current playing season to complete the mandatory adult SafeSport Core Training before any on-ice participation.
5. Dues and Registration
To be eligible to participate, a player must be in good financial standing with the club, and must have registered and provided all forms, including SR8, Report Cards, proof of birth and citizenship (for new players to the Club,) etc. before any player may participate in any on-ice activity.
International players: Non-U.S. citizen players must apply for an official international transfer through USA Hockey. The process requires approval before a player can join team practices or games, and the requirements vary depending on whether the player is coming from Canada or an international federation.
6. GPA Requirement
Players must be academically eligible to play by achieving at least a 2.0 grade point average in the marking period preceding the opening of the hockey season and must maintain that average throughout the season, and have no more than 1 failed course grade. Academic eligibility is determined on the date report cards are issued and remains until the next report card is issued.
The Club Director or Team Manager will determine proof of academic eligibility. Report cards are to be sent to the Club Director within 24 hours after they have been posted. Players MUST NOT PARTICIPATE in a game or practice if his/her Marking Period GPA is below 2.0 or there is more than one failed course grade. Instead, the player must immediately notify the head coach of the situation. Parents are asked to ensure this occurs. Failure to do this may result in expulsion from the Club because the MSHL takes these violations very seriously.
7. Team Placement (Varsity, JV, Dual)
The coaching staff will determine which players will play on the Varsity team, the Junior Varsity team, or be dual-rostered based on skill level and team make-up.
In certain situations, the Montgomery Hockey Conference (MHC) has the authority to determine that a player is “too good” for JV and therefore can only play on a Varsity team. While these situations are limited, each year it seems to come up. It causes issues for the player, the coaches, and the Club. The MHC can make this determination at any time during the playing season which makes it even more troublesome. Should this occur, the coaches will work with the player and parents to figure out what is best.
8. School Suspensions
A student may not participate in practices or contests when serving an in-school or out-of-school suspension. The student becomes eligible to participate on the next school day following the suspension.
9. Participation in WCHS Interscholastic Sports and Other Activities
Students may pa rticipate in interscholastic sports (such as WCHS football, wrestling, lacrosse, etc.) while also playing hockey with the WCHS Ice Hockey Club, PROVIDED approval has been given by the appropriate coach of each sport PRIOR to the start of each sport’s season. Many players have successfully participated in WCHS football, track and field, lacrosse, etc. while also playing Club hockey. The key is getting approval from each sport’s head coach. The Club also recognizes that players may belong to other school related clubs or activities such as Band, Theater, etc. Advanced notification must be given to the coaches at the start of the year of potential conflicts and players are responsible for working out an agreement between the coaches and the teachers/leaders of the other clubs or activities.
B. Participation Expectations and Standards for Players, Parents, and Spectators
Participation in the high school hockey league is a privilege. Accordingly, students must meet certain standards in order to earn the privilege of participation. At a minimum, the following standards are required of all student-athletes:
1. Abstinence from all Illegal Substances/Alcohol/Tobacco/Steroids/Controlled Substances
MCPS, MSHL and WCHSHC regulations state that any student-athlete with verified use, distribution, or possession of alcohol, tobacco, e-cigarettes, illegal drugs, and/or controlled substances on school property, at a school-sanctioned event, or at any WCHSHC game, practice or event is prohibited..
2. Assumption of Risk
Participation in athletic activities often includes intense competition and poses the potential for serious, catastrophic, or life-threatening injury. Participants and parents/guardians are urged to consider that there are inherent risks and hazards associated with athletic participation. Risks vary from sport-to-sport and can occur under direct supervision and with use of proper safety equipment.
3. Behavior
Demonstrate behavior that promotes and reflects the R.A.I.S.E. core values of MCPS athletics and the WCHS Hockey Club. Maintain academic and financial eligibility as outlined by MCPS, MHC, MSHL and the Winston Churchill Ice Hockey Club rules and regulations. Exhibit public behavior that will reflect positively on the WCHS Hockey Club, team, school, and community. Exert efforts to maintain a high level of academic achievement within the student’s abilities. Comply with all WCHS Hockey Club, team, school, and school system rules, regulations, and policies
Students must attend all of their scheduled classes in order to participate in a practice or contest on that day. If the head coach or designee grants an excused absence in advance for a prescheduled activity or an unforeseen emergency, the student may participate on that day.
If during the season a student has an unexcused absence, the student may not compete in the next contest after the violation has been verified, unless approved in advance of the contest by the head coach.
Exhibit responsible, respectful, and trustworthy behavior to teammates and coaches. Treat your teammates, coaches, and others with respect. Listen when coaches are speaking and be open to their constructive criticism. Be on time for all practices, events and games. All players are expected to be at all on-ice practices and at all off-ice workouts, unless excused in advance by the Head Coach. If a player has to miss a practice or any other scheduled event, it is the player's obligation to personally notify the Head Coach and Team Manager as far in advance as is practical, and provide an explanation of why the practice or team event must be missed. The Coach will determine if the absence is excused or not excused.
Maintain a positive attitude, play and practice with a high level of motivation and effort and offer positive encouragement to teammates.
Respect and comply with decisions made by the coaches, managers, and contest officials.
Attend all team functions unless ill or given prior permission to be absent by the coach. Help with team fundraisers during the year.
4. Communication with Coaches
Parents/guardians should not attempt to address coaches immediately after contests and practices. Coaches have many post contest/practice responsibilities, including supervision of players. Also, the post-contest/practice period is often emotionally charged and not conducive to productive discussion. If a parent/guardian feels a need to communicate a concern, the parent/guardian should contact the Head Coach and/or WCHS Hockey Club director to arrange a later meeting.
5. Electronic Communications
Parents/guardians and students are expected to utilize appropriate, positive use of social media, e-mail messages, blogs, websites, and other electronic communications. Parents/guardians and students shall not make inflammatory or derogatory comments and/or post inappropriate descriptions or pictures regarding students, staff members, coaches, and/or other teams or schools. Players or parents may not create any accounts, or post any items, that imply, or likely may be inferred, to be accounts run by the WCHS Hockey Club or posts made by the WCHS Hockey Club.
6. Disciplinary Actions
The following infractions by a player will be sufficient cause for disciplinary action that could result in dismissal from the Club with no refund of dues or a parent/guardian or other associate being prohibited from attending practices, games or Club events. This list is not all-inclusive.
a) Fighting, any physical, verbal, or other abuse, or harassment of a coach, manager, club official, rink employee, other parents, volunteers, security personnel or game officials. Causing disruptions or outbursts at Club meetings, events, practices or games.
b) Violation of any of USA Hockey, MSHL, WCHSHC Parent, Player or Spectator Codes of Conduct and/or a violation of any SafeSport rule. Insubordination, including the use of offensive or abusive language or jestures, or showing disrespect to a coach, team representative, player, game officials, rink employees, opponents, club volunteers, or other parents.
c) Use, Possession or Distribution of Illegal Substances/Alcohol/Tobacco/Steroids/Controlled Substances. MCPS, MSHL and WCHSHC regulations state that any student-athlete with verified use, distribution, or possession of alcohol, tobacco, e-cigarettes, illegal drugs, and/or controlled substances on school property, at a school-sanctioned event, or at any WCHSHC game, practice or event is prohibited from participating in athletic activities for a minimum of 10 consecutive school days (including any intervening non-school days). The student-athlete will be eligible for athletic activities on the eleventh school day. Similarly, the WCHSHC, may prohibit a player from participating in any games, practices or other team events for similar infractions for a shorter or longer period of time depending on the circumstances.
d) Parents or others, providing any member of any team with alcohol or any controlled or illegal substance.
e) Failure to meet financial obligations in a full and timely manner.
f) Stealing or unauthorized use of any property or equipment.
g) Destruction, defacing or damage to any property, including rink and locker room or other facilities, and/or club, teammate or competing team property.
h) Repeatedly missing scheduled games or practices without being excused in advance by the head coach.
i) Any behavior that is unbecoming to the ideals and mission of the WCHSHC, the school or the league.
j) Bringing firearms, ammunition, explosives, knives or any potentially dangerous items to any Club function.
k) Hazing or any other harassment of others.
l) Inappropriate use of social media. IMPORTANT: Players and family members may not use their own social media account(s) to post anything in a way that may represent to others that their post is an official post from the Hockey Club.
m) Bringing visitors into the locker room without prior permission of the coach or team manager.
n) Violations of any USA Hockey or SafeSport rules.
o) In addition to other infractions, a student may be suspended or removed from a team for unexcused absences or chronic tardiness to team practices, games or events.
7. Dues
The WCHSHC must pre-pay many expenses at the start of the season for league fees, ice time, officials, security, equipment, insurance and similar things. The calculation of dues is based on the number of players that sign-up for the team and the estimated costs of running the hockey program for the season. Most fees that WCHSCH must pay in advance are nonrefundable. Once they are paid by the Club, the Club is not subject to refunds due to weather, or other events that disrupt practices or games.
As a result, once the season starts players are not subject to a refund of any dues as a result of moving out of the area, sickness, injury or other reasons.
However, should a hardship situation occur, the WCHSHC board will, on a situation-by-situation basis, consider the circumstances to determine if an accommodation of some sort might be possible.
8. Eligibility status changes
Report to the Club Director (wchs_hockey@outlook.com) and the head coach any issues or developments that may affect eligibility status as soon as you are aware of them.
9. Equipment Requirements
Players must wear and keep in good condition all USA Hockey-required equipment, including mouth guards, unmodified helmets, neck protection, etc. A player may not play in a game if they do not have their Game Uniform or other mandatory equipment. The Club will purchase home and away jerseys, a practice jersey, home and away game socks, and a pant shell on behalf of each player through a team order and the player must reimbursement the Club for the cost of each item. The items become the property of the player.
10. Game Expectations
Unless school schedules make it impossible, each player is required to be at the rink on the day of the game at least 45 minutes before the scheduled start time. Out of respect for the game and player self-respect, coaches may require players to wear collared shirt, tie and slacks or similar apparel to games. Players are expected to warm up with the team and arrive in time for coach instructions. Failure to arrive in time or not appropriately dressed could result in loss of ice time for the player during the game. As a courtesy to the coaches who have to make up the lines for each game, any player who will not be at the game must tell the coach at the preceding practice or a minimum of 12 hours before the game is scheduled AND UPDATE TEAMSNAP. Failure to show up at a game without notification may result in the player sitting out the next game.
Players, parents and coaches must conduct themselves in a sportsmanlike manner at all games; before, during, and after play in compliance with USA Hockey and Safesport requirements.
Parents and spectators are expected to conduct themselves with decorum and dignity or they will be asked to leave the rink. On-ice officials, rink personnel, security personnel, coaches and club representatives may ask any spectator to vacate the arena who is being disruptive or violating USA Hockey Spectator Code of Conduct.
11. Hazing Prevention
Hazing is strictly prohibited at all times. Hazing involves any act that subjects teammates to mental or physical discomfort, embarrassment, harassment, or ridicule. In some instances, hazing constitutes a criminal act. At a minimum, hazing may lead to immediate dismissal from a team. All stakeholders across the MSHL and members of the WCHSHC are expected to exhibit behaviors that promote a positive culture.
Students or parents should immediately report any suspected hazing incident to the WCHS Hockey Club Head Coach (coachlondon-wchshockey@proton.me) and the WCHS Hockey Club director (wchs_hockey@outlook.com).
12. Health and Safety
Parents/guardians and students are required to review health and safety related information provided on the MCPS Athletics website: http://www.montgomeryschoolsmd.org/departments/athletics and the USA Hockey website: https://www.usahockey.com/playersafety These websites include information on MRSA, heat acclimatization, hydration, head injuries and concussions, steroids, sudden cardiac arrest, promoting a positive culture, and supervision.
13. Hockey Suspensions
Suspended players may be required to attend practices and games as a spectator or as a dressed non-participating player, as determined by the coach.
14. Inclement Weather Policy
The team plays and/or practices as scheduled unless notified by the coach, team manager or other club official. Notification is generally by TeamSnap, telephone, text or email and you will be told at the start of the season what methods communication will be typically used by the team.
That said, safety is always of prime consideration and it is each parent’s responsibility to decide whether the road or weather conditions are safe for their player. No player will be disciplined for not showing up at a game or practice if weather or road conditions are deemed unsafe by the parent, however the player is still responsible for notifying the coach or team manager that he or she will not be at the game or practice.
In the event of a formal cancellation, games and practices will be made up at a suitable time and place, if doing so is possible, given available ice time, officials, etc.
15. PARENT/GUARDIAN VOLUNTEER REQUIREMENT
The club requires the active participation of the parents and guardians. Without their support, there would be no Club. Parents/Guardians, as well as relatives and friends, are encouraged to attend all games and other activities.
By having a player on the team, parents/guardians are required to assist in any one of many various ways. A few examples include:
- Team manager or assistant team manager
- Off-ice official to operate the game clock or manage the scoresheet, penalty box attendant, safety monitor, or crowd control monitor
- General club volunteer
- Arranging for team refreshments or snacks
- Hosting team dinners
- Photographing or videotaping the games
- Managing a club merchandise store
- Creating the club’s yearbook ad
- Tutoring students in need
- Serving on the senior night or banquet night committee
- Sponsoring the team, jerseys, or supplies

16. Photography and Videography
Parents and players acknowledge that the WCSHC, WCHS, MHC, MSHL or public news & sports outlets may use photos or videos taken as part of club activities and share them for the purposes of publicizing the hockey team via emails, game brochures, yearbooks, public websites, etc. Should there be any concern with your particular player, then at the start of the season contact the WCHSHC by emailing: WCHS_Hockey@outlook.com
17. Practice Expectations
A practice schedule will be established prior to the start of the season. It is anticipated that both teams will practice 1-2 times per week at Cabin John and/or Rockville rinks.
All players are required to show up to practice. If a player cannot attend a practice, the coach or team manager must be notified by phone or e-mail as soon as the player knows they will not be attending.
Unless directed otherwise, PLAYERS ARE TO USE TEAM SNAP TO INDICATE THEIR INTENT TO ATTEND EACH GAME, PRACTICE, TEAM DINNER OR OTHER EVENTS AT LEAST 72 HOURS BEFORE THE EVENT. FAILURE TO KEEP TEAMSNAP UPDATED MAY RESULT IN A PLAYER BEING BENCHED OR SUSPENDED FROM THE TEAM/CLUB AND DUES WILL NOT BE REFUNDED.
Excused absences are permitted for illness or academic commitments with prior notification to the coach from the player or his/her parents. Excused absences, at the coach’s discretion, may affect a player’s game ice time, due to missing key instruction at the practice. Each unexcused missed practice may result in loss of ice time during the next game.
18. Privacy and Use of Player, Parent and Family Information
In order to properly and efficiently manage the overall ice hockey program, the WCHSHC requests information such as has been done in the registration package.
Parents and players are reminded that their access to any information they may come across of any other player or player's family as a result of their association with WCHSHC, such as through TeamSnap, text message groups, group emails and similar venues, is strictly for the coordination of team and Club activities while their player is on a WCHSHS team. Parents and players may not use this information for non-Club business or personal or commercial purposes or further publish or disseminate this information.
Further, player and parent information, such as but not limited to name, telephone numbers, addresses and USA Hockey Registration number may be included in TeamSnap, Google Sheets, GameSheet or other mechanisms necessary for the effective functioning of the Club and/or league, and that information may be accessible by other Club families, league officials, and others supporting Club or league activities.
19. TeamSnap Responsibilities
PLAYERS ARE REQUIRED TO USE TEAM SNAP TO INDICATE THEIR INTENT TO ATTEND EACH GAME, PRACTICE, TEAM DINNER OR OTHER EVENTS AT LEAST 72 HOURS BEFORE THE EVENT. FAILURE TO KEEP TEAMSNAP UPDATED MAY RESULT IN A PLAYER BEING BENCHED, OR SUSPENDED FROM THE TEAM/CLUB, AND DUES WILL NOT BE REFUNDED. Parents and players are to update the contact information in TeamSnap to ensure coaches and managers can readily contact players and parents/guardians.

Stipulations
We have received and read the STUDENT/PARENT/GUARDIAN ATHLETIC PARTICIPATION CONTRACT AND PARENT PERMISSION FORM. Based on this information, we understand and stipulate to the following. I/We:
1. Understand the eligibility regulations required for participation and affirm that all eligibility requirements have been satisfied, including age, residence, and academics, and, should any of that change during the school year, will immediately notify the Club via an email to wchs_hockey@outlook.com of the changes.
2. Understand that participation of ineligible players will result in individual and team sanctions, including forfeits for the team.
3. Affirm that the student will exert effort to maintain a high level of academic achievement.
4. Understand that there is potential for serious, catastrophic, or life-threatening injury associated with participation in a sport; understand that the sport in which our child will be participating is potentially dangerous, and that physical injuries may occur to our child requiring emergency medical care and treatment; and assume the risk of injury to our child that may occur in an athletic or team activity.
5. Understand and accept that the MCPS, the MSHL, MHC and the Winston Churchill High School Hockey Club, do NOT provide any athletic trainers or medical personnal at any WCHSHC events, activities, practices, or games.
6. Acknowledge the availability to us of safety and health information made available by USA Hockey, and/or MCPS, including information regarding concussions, MRSA, hygiene, heat acclimatization, hydration, steroids, and sudden cardiac arrest.
7. Affirm that the student shall not participate in hazing at any time, of any nature, and will immediately report any suspected hazing or incidents or inappropriate behavior to the WCHS Hockey Head Coach (coachlondon-wchshockey@proton.me) and the WCHS Club director (wchs_hockey@outlook.com).
8. Shall exhibit, as a participant or spectator, a high level of sportsmanship at contests, which promotes and reflects the R.A.I.S.E. core values of MCPS athletics, USA Hockey and the WCHS Hockey Club.
9. Shall follow appropriate procedures in communicating concerns to coaches.
10. Affirm that the student will abide by all Participation Expectations and Standards, and failure to do so may result in diciplinary actions including being released from the WCHS Hockey Club with no refund in dues paid or relief from dues owed.
11. Shall utilize appropriate, positive use of technology, including social media and other electronic or other communication mediums; shall not represent themselves or in any personal electronic or other media as being that of the Winston Churchill High School Hockey Club; and shall not solicit funds, donations or sell or giveaway any merchandise that in any way is a representation, or could reasonably be inferred to be a representation, of the Winston Churchill Hockey Club.
12. Affirm that the student shall not use steroids, illegal drugs, alcohol, e-cigarettes, or tobacco unless medically prescribed for a specific condition or illness.
Permission to Participate
Players and parents, by signing below, acknowledge that they have read, understand, and will obey the Student Eligibility Requirements and Participation Expectations and Standards for Players, Parents, and Spectators. I/we:
- Agree to obey and follow all rules and regulations of USA Hockey, its affiliate the Potomac Valley Amateur Hockey Assoc. (PVAHA), the Maryland Scholastic Hockey League (MSHL), the Winston Churchill High School Hockey Club, Inc. (WCHSHC), each ice facility used by the WCHSHC, and Winston Churchill High School and MCPS rules related to its student athletes.
- I/We understand that the WCHSHC is not a part of Winston Churchill High School or MCPS, and the WCHSHC follows the applicable guidelines outlined by MCPS for student-athletes.
- Hereby authorize and consent to our child’s participation as a player on a team organized by the the Winston Churchill High School Hockey Club, Inc. (WCHSHC), and my child has my/our permission to participate in hockey and associated activities, such as off-ice drills, training, and similar activities while participating with the WCHSHC.
- In consideration of the acceptance of our child by WCHSHC, Inc. into its program, and the benefits derived by our child from participation, agree to release and hold harmless the WCHSHC Inc., its members, managers, directors, coaches, and any and all other of their volunteers, agents, servants, and/or employees, as well as entities that provide training to WCHSHC coaches and/or athletes as part of the athletic program, and agree to indemnify each of them from any claims, costs, suits, actions, judgment, and expenses arising from our child’s participation in the program.
- Hereby give our consent and authorize the WCHSHC and its volunteers, managers, coaches, agents, servants, and/or employees to consent on our behalf and on behalf of our child, to administer emergency medical care and treatment in the event we are unable to be notified by reasonable attempts of the need for such emergency medical care and treatment.
- Have responded truthfully and accurately to the questions in the Residency portion of the Contract.
- Understand and conform to all of the statements in the Stipulations portion of the Contract
Please note: In the event that both parents or two guardians retain legal guardianship of the student, the signatures of both parents or guardians are required.
