  If you live within 20 miles of Wurtsboro you'll be able to pick up your prize locally. Those who live further can choose to have winning items shipped for no additional charge.

3. Double check the total cost of all of your tickets above the "Submit" button.
4. Click "Submit". Jotform will automatically send you a confirmation email within a few minutes.
