ROSEWOOD CANINE Acknowledgement of Risk: I understand that participation in dog training involves inherent risks including injury to myself, my dog, or others. I voluntarily assume all risks associated with participation. Responsibility for My Dog: I agree to maintain control of my dog at all times and accept financial responsibility for any injury or damage caused by my dog. Health & Vaccinations: I certify my dog is healthy and current on required vaccinations, Flea & Tick prevention, and free of contagious illness. Behavior Disclosure: I have disclosed any known aggressive or dangerous behaviors. Rosewood Canine may remove any unsafe dog from class or group activities. I understand that I am required to keep my dog quiet to keep a quiet, peaceful environment. Release of Liability: I release and hold harmless Rosewood Canine, its owners, staff, and representatives from all claims related to participation. Indemnification: I agree to indemnify and defend Rosewood Canine from claims arising from my dog’s actions. Emergency Veterinary Authorization: No Guarantee of Results: Training outcomes vary and no specific results are guaranteed. Dogs must remain controlled and owners must follow.
