Emergency Information

FESTIVAL LIABILITY WAIVER & RELEASE

Instructions: This waiver must be completed for every individual participant attending the OKC Tap Fest.

• Adult Participants (18+): Please read each section below and enter your own initials to acknowledge and agree to the terms.
  
  
• Participants Under 18: A parent or legal guardian must read each section and enter their initials to acknowledge and agree on behalf of the minor student.

By initialing the sections below, the signing individual (or parent/guardian) acknowledges that they have fully read, understood, and agreed to all terms set forth in each paragraph.

I hereby release the OKC Tap Fest, Metropolitan School of Dance, Inc. (MSDI), the instructors, artists, sponsors, affiliates, promoters and facility management from any and all claims of liability due to personal injury or loss of property which (my child or I) may sustain as a result of participating in an activity connected with OKC Tap Fest. In addition, I acknowledge and understand that the teaching faculty and class schedule may be subject to change.  

I hereby release the OKC Tap Fest, Metropolitan School of Dance, Inc. I state that I (and my child) will comply with the rules and regulations set up by the OKC Tap Fest and MSDI. I further acknowledge and understand that both OKC Tap Fest and MSDI have the right to refuse service to any person at its own discretion.

I agree to allow the OKC Tap Fest and MSDI to use any video or photographic pictures taken, of myself (or of my child), for any promotional and/or archival purpose. Please leave blank if you do not want your or your child’s pictures shared publicly. We will honor this decision.

Teacher Discount - Teachers, we value you and what you do for your students. As our Thank You to you, for every student that registers and lists you as their teaching instructor, you will receive a $10 credit that goes towards your OKC Tap Fest registration. Make sure they share your name on the question above “How did you hear about OKC Tap Fest?” Teachers, please alert us after all of your students have registered before paying your fee.
