By filling out this form, you acknowledge the following

Consent

You hereby give your approval for your / your child's participation in any and all activities prepared by ATTAQWA MUSLIM SCOUTS during the Wild Adventure Camp. In exchange for the acceptance of your / your child's candidacy, you release, absolve and hold ATTAQWA MUSLIM SCOUTS harmless and all its respective leaders and volunteers, from any and all liability for injuries to you / your child arising out of participating in the selected activity session. In case of any injury to you / your child, you hereby waive all claims against ATTAQWA MUSLIM SCOUTS including all leaders, all volunteers, and all participants.
Property Use Agreement

By registering for and participating in the camp, you acknowledge and agree that all property, facilities, and equipment provided by ATTAQWA MUSLIM SCOUTS (including but not limited to tents, tools, and other camping gear) are to be used solely at your own risk and under your own responsibility. You further acknowledge and accept full liability for any loss of, or damage to, ATTAQWA MUSLIM SCOUTS property resulting from misuse, negligence, reckless conduct, or failure to adhere to provided instructions or guidelines.In the event of any such damage or loss, a monetary charge may be imposed to cover the repair or replacement cost, at the discretion of ATTAQWA MUSLIM SCOUTS. All participants are expected to treat all items and facilities with due care and respect at all times.
