Liability Waiver and Release
By signing this form, I acknowledge and agree to the following terms as a participant (or parent/guardian of a participant) in The Waveffect Academy and the WESSA programs, events, and activities:
1. Assumption of Risk:
I understand that participation in basketball and related activities involves inherent risks, including but not limited to physical injury, illness, or accidents. I voluntarily assume all such risks, whether anticipated or unanticipated.
2. Release of Liability:
I agree to release and hold harmless The Waveffect Academy, WESSA, their coaches, staff, volunteers, affiliates, sponsors, and facilities from any and all claims, demands, causes of action, or liability for injuries, damages, or losses arising from participation in activities associated with the league or academy.
3. Medical Authorization:
In the event of an emergency, I authorize The Waveffect Academy and its representatives to obtain medical treatment deemed necessary for the participant. I agree to assume responsibility for all associated medical costs.

Media Release Consent
I hereby grant The Waveffect Academy and WESSA permission to photograph, film, or record the participant during events, practices, and related activities. I understand that these images and recordings may be used for promotional materials, including but not limited to:
Social media platforms
Website content
Flyers, brochures, and other marketing materials
Newsletters and press releases
I waive any right to inspect or approve the final materials and release The Waveffect Academy and WESSA from any claims related to the use of these materials.
