WELCOME TO THE ROCKETS HOCKEY CLUB
2026-27 Player Equipment Reorder Form

Use this form to purchase additional apparel and equipment items included in your player’s 2026-27 equipment package. The form will display only the items available for the team associated with your player’s profile.

STEP 1: Review Sizing
Review the Jersey & Apparel Sizing Charts and use your player’s current gear as a reference.

STEP 2: Auto-Populate the Player Profile
Enter the player’s full name exactly as it appears on the signed contract, then select “Click To Auto-Populate Form.” If the player or team does not populate correctly, stop and email equipment@rocketssportsgroup.com.

STEP 3: Select Replacement Items
Select one or more eligible replacement items. The player’s original equipment-package items will appear, and Workout Shirts and Workout Shorts are available to all teams.

STEP 4: Select the Requested Size
Choose the requested replacement size for every selected item.

STEP 5: Review, Pay, and Submit
Review the order total, complete the Stripe payment, sign the acknowledgment, and submit the form.

LIMITED AVAILABILITY: Most apparel and equipment inventory is in stock, but limited and requests are fulfilled first come, first served based on completed paid submissions. Game jerseys requiring names are custom orders and may take up to 60 days.
