- As the parent or legal guardian of the player(s) named above (the “Player”), I allow them to fully participate in all activities of the Ascend World Academy training camp and affiliated programs (Ascend Volleyball, Basketball, Soccer, & Handball). I understand that participation in the training involves certain risks, including the possibility of serious bodily injury, illness (such as COVID-19), damage to personal property, or even death. On behalf of myself and the Player(s), I acknowledge these risks and voluntarily accept them. I waive any right to pursue legal action and release Ascend World Academy their affiliates, directors, officers, staff, agents, volunteers, and employees from any claims or liabilities arising from or related to the Player(s)’ participation in the Program. Additionally, I agree to indemnify and hold harmless Ascend World Academy from any claims, actions, or demands made by the Player(s) or on their behalf related to their participation in the Program.
  
  I have provided full disclosure of any medical conditions or allergies that the Player(s) may have. I authorize Ascend World Academy to make necessary medical decisions on behalf of the Player(s) during the training camp. The staff will make reasonable efforts to notify me as soon as possible in the event of any medical issues.
  
  Both the Player(s) and I agree to abide by the policies of Ascend World Academy.
  
  I acknowledge that the personal information provided in this registration form is necessary for the administration of the Player(s)’ activities during the training camp. This information will be used for communication, medical treatment purposes, and for coordinating with medical providers if necessary.
