Permission Form and Medical
We, Parent/Guardian (full name)* (parent/guardian initial, over 18+) hereby give permission for our son/daughter; to travel from Calgary, AB to Ottawa, ON from August 25-29, 2026.

Our son/daughter is attending the CKC National Championships with the Calgary Canoe Club. Coaches and/or chaperones throughout the trip will supervise athletes.

I give my permission for medical assistance to be administered should they become ill or involved in an accident initial* (parent/guardian initial, over 18+).
CCC has collected information when you join the program but did not collect Health Card Numbers at that time. Please provide the athlete health card number before the trip (just in case we need a trip to the hospital).
