By participating in Pass the Beauty Inc. programs, events, support groups, or activities, I understand that participation is voluntary. I agree to follow program safety guidelines and understand that reasonable precautions will be taken to provide a safe environment.
I acknowledge that accidents, injuries, or unexpected incidents may occur and agree to promptly report any concerns to staff. In the event of an emergency, I authorize Pass the Beauty Inc. staff to contact emergency services and/or my listed emergency contact as appropriate.
I understand that this waiver does not remove any rights or responsibilities that cannot legally be waived.
