I understand parents are strongly encouraged to volunteer in the Nursery and/or Children & Youth Church at least two (2) times during each ministry year unless prevented by work, health, or other extenuating circumstances. A monthly sign-up for Children & Youth Church assistance as well as other volunteer opportunities will be communicated in advance, and every effort will be made to accommodate my family's schedule.

Eligibility for Monetary Gifts & Special Activities

I understand that students who do not regularly participate in worship services, Sunday School, Children & Youth Church, Bible Study, ministry events, and other approved Children & Youth Ministry activities may not be eligible for year-end monetary gifts, scholarships, other financial recognitions, or special activities including, but not limited to, PHD Sundays and field trips.

Please note: Special consideration will always be given to families with extenuating circumstances who proactively communicate with the ministry.

Nursery Volunteer Eligibility
I understand that children under 18 may not serve in the Nursery. Nursery volunteers must be at least eighteen (18) years of age AND have graduated from high school (or earned a GED). Eligible volunteers must also complete required background checks, Child Protection training, and ministry orientation before serving.

One Stop Check-Out Policy

The safety and security of every child entrusted to the care of the Fifteenth Avenue Baptist Church Children & Youth Ministry is one of our highest priorities. To help provide a safe, orderly, and consistent dismissal process, the Children & Youth Ministry utilizes a One Stop Check-Out procedure for all children ages 12 and under.

One Stop Check-Out Procedures
- The check-in/check-out process applies to all children ages 12 and under.
- Unless a child is already attending Sunday School immediately before Children & Youth Church, a parent, legal guardian, or authorized adult (18 years of age or older) must check the child in.
- At the conclusion of Children & Youth Church, all parents/guardians of children ages 12 and under must report first to the Children & Youth Ministry Check-Out Station located at the back door of the Activity Center.
- A member of the Children & Youth Ministry Team will verify the authorized adult and dismiss the child(ren).
Nursery Pick-Up (Children Under Age 5)

After checking in at the One Stop Check-Out Station, parents/guardians picking up children under the age of five (5) will then proceed to the Nursery to pick up their child. Parents should not go directly to the Nursery without first stopping at the Check-Out Station.

Authorized Pick-Up

If an individual who is not listed on the Authorized Pick-Up List attempts to pick up a child, the Children & Youth Ministry reserves the right to refuse release of the child until authorization has been confirmed by a parent or legal guardian, or an approved adult arrives to pick up the child.

Students Ages 13 and Older

Students 13 years of age and older may dismiss themselves at the conclusion of Children & Youth Church unless a parent or legal guardian requests that they also participate in the check-in/check-out process.

I understand that it is my responsibility to promptly notify the Children & Youth Ministry of any changes to my child's contact information, medical information, emergency contacts, authorized pick-up list, or any other information provided on this registration form. I understand that questions regarding any policy or procedure should be directed to the Youth Minister prior to signing this agreement. Finally, I look forward to partnering with the Fifteenth Avenue Baptist Church Children & Youth Ministry as we work together to help my child grow in faith, character, and their relationship with Jesus Christ.
