1. Please provide a complete, detailed narrative description of your firm's previous experience identifying and remediating housing-related health hazards. This should include staff to be utilized, experience per staff member; such as intake/outreach/contractors/etc., and the percentages of time committed for all staff identified.

2. Description of experience with similar projects undertaken in the last 3 years and contact persons to be interviewed, if necessary. Highlight any construction services involving any federal and state funded projects (especially those with lead remediation, housing rehabilitation, or weatherization.) Provide details if performance benchmarks were achieved, program oversight was addressed, and if financial management controls were successfully implemented.

3. List of current contractors used for lead remediation/home rehabilitation, weatherization, and accessibility projects. Please list any RRP/Lead/Weatherization/Accessibility certificates or licenses.
