Youth Advisory Council (YAC) Photo, Video & Media Release Authorization 2026-2027 School Year
Thank you for allowing your student to participate in the All City Music Youth Advisory Council (YAC). We are excited to support the next generation of young leaders through leadership development, mentorship, and community engagement.
Throughout the Youth Advisory Council program, All City Music may take photographs, record video, capture audio, and collect quotes that celebrate students' leadership journeys and showcase the impact of our programs.
In addition to documenting activities for internal purposes, All City Music may use these materials to share our students' success with community partners, schools, donors, grantmakers, supporters, and the public. These materials help us tell the story of our work, inspire future participants, and secure funding that allows our programs to continue serving young people.
Please read the authorization below carefully.
Parent/Guardian Authorization
I am the parent or legal guardian of the student named below.
I hereby grant All City Music permission to photograph, videotape, audio record, interview, and/or otherwise capture my student's likeness, image, voice, name, statements, and participation during Youth Advisory Council activities and related All City Music programs.
I understand that these materials may be edited, reproduced, published, distributed, displayed, or otherwise used by All City Music for nonprofit, educational, promotional, fundraising, reporting, and marketing purposes. This includes, but is not limited to:
Printed publications
Annual reports
Grant applications and reports
Newsletters
Social media platforms
The All City Music website
Presentations
Promotional materials
Digital and online media
Other forms of communication that support All City Music's mission
I understand that my student and I will not receive financial compensation for the use of these materials.
I further understand that All City Music will use reasonable care in representing my student respectfully and professionally.
Lastly, I give All City Music permission to feature my student in leadership spotlights, success stories, interviews, or testimonials that highlight the impact of the Youth Advisory Council. I understand these stories may be shared through All City Music's website, social media, newsletters, annual reports, grant materials, and presentations.
By signing below, I release and hold harmless All City Music, its Board of Directors, employees, contractors, volunteers, partners, sponsors, and representatives from any claims, demands, or liability arising from the authorized use of these photographs, recordings, interviews, or other media as described above.
This authorization shall remain in effect unless revoked in writing by the parent or legal guardian. Revocation will apply only to future use and will not require the removal of materials already published or distributed.
