A version of this form must be signed and on file for my pet(s) to be admitted for service at Howling Woods K9 LLC. Howling Woods K9 LLC, hereby referred to as "HWK9" may provide changes to its policy's at any time.

HWK9 agrees to provide services to my pet(s) as indicated on the Boarding Contract or via text message agreement. HWK9 reserves the right to exercise its best judgment in all circumstances as to how to best provide the services selected and will exercise reasonable care to ensure the safety of my pet. HWK9 will keep its premises sanitary, properly housed, and will ensure my pet is watered, fed, and aired regularly.

For the health and safety of all pets at HWK9, the following health procedures are required:

• IMMUNIZATIONS: Pets boarding with Howling Woods K9 are required to have the following immunizations performed by a certified and licensed veterinarian:
  1. Rabies (Puppies 4 months and younger excluded)
  2. Distemper
  3. Bordetella
  4. Canine Influenza (Highly recommended)
• FLEA/TICK HEARTWORM PREVENTATIVE: Pets must be on a monthly flea and tick preventative. If my pet is found to have parasites I will be notified immediately. I will be encouraged to pick up my pet, if I cannot make arrangements, HWK9 will provide any necessary treatments at my expense. If I provide preventative, HWK9 will administer at date specified by me.
• HEALTH/BEHAVIOR: HWK9 is equipped to care for older pets, administer certain medications for chronic conditions, however, they cannot care for the critically ill, aggressive, or biting pets.

HWK9 in its sole discretion may be authorized to perform first aid as deemed necessary for the health and well-being of my pet. If it is believed my pet needs emergency veterinary care, every attempt will be made to contact me first & my emergency contact. In the event HWK9 cannot get in contact with myself or my emergency contact, I authorize HWK9 to transport and release my pet to a veterinary clinic at my expense. In the unlikely, but possible, event that my pet becomes ill, injured, escapes, or deceased, despite all the supervision and precautions taken by HWK9, I hereby release HWK9 its staff, volunteers, or other, of any liability of any kind whatsoever arising from, or as a result of, my pet being in the care of HWK9.

Notice of cancelations must be given 24 hours before the start date of your requested Boarding Reservation. Cancelations less than 24 hours prior to the start date, or no show, will result in a full day charge and/or refusal of future services.

PETS NOT PICKED UP ON DEPARTURE DATE: In the event I do not pick up my pet on the scheduled departure date agreed upon on my Boarding Contract, HWK9 reserves the right to continue to provide the daily services set forth in my Boarding Contract for 5 days. After the fifth day, if I do not contact HWK9, my pet will be deemed "Abandoned" and the Abandoned Pet Procedures as outlined below, will be followed.

• Abandoned Pet Procedure: Once deemed abandoned, all serviced for the pet will be terminated. HWK9 will send a certified letter (return receipt requested) to my address provided on this document the (5th) day following the scheduled and agreed upon departure date, advising me that if my pet is not picked up within 10 days of the notification date, it will be deemed "Abandoned" and HWK9 will contact the appropriate authorities for the delivery of the animal. All fees associated herein, will be my sole responsibility. I shall remain liable to HWK9 for any and all unpaid charges, including court costs and attorney's fees incurred in the collection of these charges.

PERMISSION TO USE PHOTOGRAPHS: I grant HWK9, its representatives and staff, my permission to use photographs of my pet, and property for marketing and promotional purposes. I authorize any assignees and transferees to copyright, use and/or publish the same in print and/or electronically.

By signing this Agreement, I represent that I am the true owner of the above stated pet(s). I have read, understand, and complete this document and enter into this Boarding Agreement. I attest that all the information I have provided about myself and my pet is complete, true, and accurate to the best of my knowledge.
This Agreement will remain in effect for all my pet's visit's to HWK9.
