Thank you for your interest in a GoldenAcres puppy!
All dogs used in our breeding program have had their hips and elbows certified by OFA, have their eyes cleared annually by a board-certified ophthalmologist and have their hearts cleared by a board-certified cardiologist. Our goal at GoldenAcres Retrievers is to produce a happy, healthy, and structurally correct puppy with a "True Golden Temperament." We personally look to place our puppies into homes where they are going to be in the house and part of the family. Whether they are being placed as show dogs or as family companions, our goal is to breed the temperament and character that best suits this lifestyle. We not only strive for a temperament that is gentle, friendly and easygoing but also one that will be versatile for the show, obedience and/or fieldwork.
The completion of this puppy questionnaire will help us to place the right puppy in your home. All information provided will be kept confidential and used only by us. Thank you for your interest in a GoldenAcres puppy.

When this puppy or dog is purchased as a family companion and is not going to be bred, do you agree to the following and submit a veterinarian certificate to us as proof of surgery for our records?
- To have your female spayed after her first heat cycle (minimum age required)
- To have your male neutered between 16 and 20 months

Showing/Breeding Questionnaire
If your puppy or dog is not going to be spayed or neutered and is being sold for showing or breeding, please complete the following questions.
What is your reason for breeding?
If you decide to purchase a show quality puppy, are you willing to show it or have the breeder show it to his/her Canadian and/or American championship?
Would there be any other titles that you would be interested in and willing to put on this puppy or dog?
If this puppy is purchased for the purpose of showing and/or breeding and it is found not to be of breeding or showing quality, would you spay or neuter it upon the request of the breeder?
Are you willing to take full responsibility for all puppies produced by this dog?
At any time if a situation arises that one of the owners of one of the puppies that you have placed or sold from a litter out or by this dog is no longer able to keep and care for it, would you be willing to take it back and find it another suitable home?
Are you willing to place all puppies that are sold as family companions to pet homes on either a CKC non-breeding contract or a AKC limited registration and follow up to ensure they are spayed/neutered?
Do you agree that any puppies that are being sold with the potential to be bred will include the following in your contract:
- The dog or bitch will be a minimum of 24 months prior to breeding
- The dog or bitch will have their final OFA hip and elbow clearances before breeding

- The dog or bitch will have a current eye clearance from a board-certified ophthalmologist (current every 18 months) and a heart clearance from a board-certified cardiologist before breeding
- That they will be of breeding quality, including temperament and that the owner has endeavoured to obtain its championship or another CKC/AKC title.
It is our policy at GoldenAcres Retrievers to place a dog on a co-ownership if that puppy or dog is being placed in a show/breeding home and is not going to be spayed or neutered. Would this be a concern for you and if so, please state any concerns you may have with a co-ownership with GoldenAcres Retrievers.

Thank you for taking the time to fill out this questionnaire to aid in us finding the right puppy for you. Please feel free to call or email if have any questions or would like more information. If you would like to come and visit us and the dogs, we would be more than happy to set up an appointment. We encourage you to book a meet and greet as it gives us a chance to ask you a few more questions and understand what you are looking for in your new family member. When placing our puppies, we do not work on a first-come, first pick basis, but rather look at each family and their home lifestyle and match the right puppy with you. We do take your considerations into account when choosing the puppies for their new homes, but the most important thing for us is to place the right puppy with you. We have found over the past decade that this is the best way, and hope the puppy that is chosen for you is a welcome addition to your family. If we have a current litter due or on the ground and you are committed to a new puppy or dog to join your family, we will require a $500.00 non-refundable deposit once your questionnaire is

accepted. This assures us of your commitment and that you are ready and have seriously thought about a new member joining your family. If, for any reason, there is not a puppy available from the litter, then the deposit will be returned to you. Thank you for your interest in a GoldenAcres puppy!
