I/We understand that dance classes, including the audition process, may include, without limitation, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I/We further understand that all the activities of the dance class involve some degree of risk of strain or bodily injury. Equinox Arts Academy, Inc. is not responsible for any injuries or damages experienced by me/my child during her/his participation in such activities. I/We also release and hold harmless the Studio, its officers, directors, agents and employees, and those acting under its authority, from all actions, claims and liabilities relating to me/my child's participation in any and all programs.

I/We consent and give permission to the Studio, and those acting under its authority, to use the name or likeness of my child for the purpose of marketing the studio.

I/We give permission for my child to participate in the 2026-2027 Equinox Arts Academy, Inc. Company auditions. I understand that all tryout practice and auditions are closed to spectators and that all decisions are final. Scores are proprietary information and are not shared with dancers and/or parents. Audition evaluations, scoring, and selection criteria are confidential and will not be released or discussed.
