AUTHORISATION OF CONSENT
Our service uses Coles brand 50+ sunscreen cream. If you would prefer a different sunscreen to be applied you are required to provide the sunscreen. The sunscreen needs to be 50+ or higher, cream based and within the expiry period. We also require the sunscreen to be in original packaging and named with the child’s full name and in the child’s bag each time they attend the service.
By signing this application of Sunscreen cream, I give permission for the Educators to administer the sunscreen cream supplied by parent/guardian, and to be applied in accordance with services Sun UV Safe policy. (Available on our website www.parkorchards.org.au)
IMPORTANT: This authorisation is for a personal preference to a sunscreen other than Coles brand 50+, if your child has an allergy to sunscreen you need to contact us and complete an allergy action plan.
