Equipment Liability Agreement

Party Time Gives Back provides party equipment at no cost to your family through the generosity of donors, volunteers, and community partners. By accepting this assistance, you agree to use all equipment responsibly, supervise children at all times while the equipment is in use, and return all items in the same condition they were received, excluding normal wear and tear.

The applicant is responsible for ensuring the equipment is not misused, damaged through negligence, altered, or removed by unauthorized individuals. Failure to return equipment, intentional damage, or misuse of any equipment may result in reimbursement for replacement or repair costs and may make the applicant ineligible for future assistance through the Party Time Gives Back program.

This agreement helps us continue serving other families in our community by protecting the donated resources that make this program possible.

Damage Reimbursement Policy

Party Time Gives Back is funded through donations, volunteers, and community support. If any damage is caused to the venue, equipment, or property as a result of negligence, intentional misuse, vandalism, or failure to follow safety instructions by the applicant, guests, or anyone attending the event on the applicant’s behalf, the applicant agrees to accept financial responsibility for the cost of repair or replacement.

Normal wear and tear is expected and will not result in reimbursement. However, damage caused by misuse, abuse, or failure to properly supervise participants may require reimbursement and may result in the applicant becoming ineligible for future assistance through the Party Time Gives Back program.

This policy helps ensure that donated equipment and community resources remain available for other families in need.

Cancellation Policy

Party Time Gives Back is made possible through donations, volunteers, and community support. Because supplies are purchased, equipment is reserved, and volunteers commit their time specifically for your child’s celebration, we ask that you notify us as soon as possible if you are unable to keep your scheduled party.

Families must cancel at least 72 hours before their scheduled event. Cancellations made with less than 72 hours’ notice, failure to respond to confirmation attempts, or failure to be present on the scheduled event date may result in the family becoming ineligible for future assistance through the Party Time Gives Back program.

Exceptions may be considered only in the event of a true emergency and are at the sole discretion of Party Time Gives Back.

Fraud Acknowledgment

Party Time Gives Back is a community-funded program created to assist families who are experiencing genuine financial hardship. By submitting this application, you certify that all information and supporting documentation provided is true, complete, and accurate to the best of your knowledge.

Any false statements, altered or fraudulent documents, omitted information, or intentional misrepresentation of your circumstances may result in immediate disqualification from the program, cancellation of any approved services, and permanent ineligibility for future assistance through Party Time Gives Back.

By signing this application, you acknowledge that Party Time Gives Back reserves the right to verify the information provided and to deny or revoke assistance if fraudulent or misleading information is discovered at any time.

Verification Consent

To ensure that Party Time Gives Back assistance is provided fairly and to families who genuinely qualify, you authorize Party Time Gives Back to verify the information submitted in your application. This may include contacting employers, landlords, schools, government agencies, service providers, or other individuals or organizations, when necessary, to confirm the accuracy of the information you have provided.

You understand that verification is used solely to determine eligibility for assistance and that all personal information will be handled with respect and used only for purposes related to your application.

Refusing to provide requested information or authorizations, or preventing the verification process, may result in delays, denial of assistance, or disqualification from the Party Time Gives Back program. By signing this application, you certify that you have read and understand this verification process and consent to the verification of the information you have provided.
