Bear Care Clinic provides health care for children of all ages who live in or attend White Bear Lake Area Schools.

All children can receive care at Bear Care Clinic, regardless of insurance status or ability to pay.

Patients age 18 and older must complete this form themselves. A parent/legal guardian must complete this form for patients under age 18, unless the parent/legal guardian has given prior permission for the student to make an appointment themselves. 

If all same-day sick visits are taken online, please call us and we will do our best to fit you in.

Before the Sports Physical Appointment

If possible, complete the sports physical form before the appointment and bring it with you. If you cannot print the form, we can provide one at the clinic.

For patients under age 18, a parent/guardian should either:

Attend the appointment; or
Be available by phone during the appointment and complete the required sports physical information before the visit.
Minnesota school sports physicals are generally valid for three years.

If the patient recently had a well-child exam at their primary care clinic, that provider may be able to complete the sports physical form without an additional examination.

Our mission is to provide accessible, affordable care to youth.

• All children can receive health care services at the clinic, regardless of their ability to pay or health insurance status. 
• You will never be sent to collections for your inability to pay a clinic bill.

There are options for patients who have difficulty paying the bill.

1. Any payment received is considered payment in full. OR
2. If you are unable to pay, your bill will be covered by the Rise Up Health Clinics Health Equity Fund.

Consent for Health Services

By signing this form, I agree to the following:

• I authorize Rise Up Health Clinics (which operates the Bear Care Clinic) and its staff to provide necessary examinations, treatments, procedures, and care as deemed appropriate by the healthcare providers.

• I have the right to refuse or discontinue treatment or care at any time. However, I acknowledge that doing so may have potential risks or consequences, which your healthcare provider will explain. 

• If I have any questions about the care provided, I will ask the healthcare provider. 

• I consent to receive communications from the clinic, including appointment reminders, follow-up calls, and billing inquiries. 

• Rise Up Health Clinics may use student health records to evaluate the quality of care and program effectiveness. 

• The patient's school may give class schedule and immunization information to Rise Up Health Clinics only when necessary to provide patient care.
  Rise Up Health Clinics may share immunization information only with the patient's school.

• Immunization information may be shared through the Minnesota Immunization Information Connection (MIIC) with healthcare providers, schools, health departments, and others authorized under law to receive it. 

• If receiving communicable disease testing, I acknowledge that the results are required by law to be sent to the Minnesota Department of Health for reporting.
