I agree that the above typed information is correct to the extent of my knowledge and that I have read all the information to follow:
1. I agree to be careful and abide by safety rules and code of conduct pertinent to the occasion.
2. I agree to release The Blooming Lady and associated parties from any legal or financial action in the chance of an accident, injury, or allergies that might result during class, on the premises, or after departing.
3. The Blooming Lady or any of its associated parties are not responsible for lost, missplacement, or expiring products after leaving the course.
4. In the case of broken utensils, tools, or spilled materials; the person responsible will be charged a loss fee depending on the item broken. Fee will vary from $5-$40 in accordance with the value of the item broken.
5. A (5) day notice is required as a courtesy in case I am unable to attend the class, getting a 50% of the payment back. The other 50% of the payment is considered a non-refundable deposit. No-call, no-show will not be refunded.
