By signing below, I hereby grant DC Cheer permission to photograph and/or record video of my child during the 2025 Cheer Clinic. I understand and agree that these photographs and recordings may be used by DC Cheer for promotional and informational purposes, including but not limited to social media, marketing materials, and organizational publications. I acknowledge that I will not receive compensation for the use of these images or recordings and release DC Cheer, its representatives, and affiliates from any claims, demands, or liability related to their use.

By signing below, I acknowledge that participation in the 2025 Cheer Clinic involves inherent risks, including but not limited to physical activity and related injuries. I hereby assume all risks associated with my child’s participation and agree to release, indemnify, and hold harmless DC Cheer, its coaches, volunteers, staff, and affiliates from any and all claims, liabilities, damages, or expenses that may arise from participation in this event, including those resulting from negligence, to the fullest extent permitted by law.

Please remit the $35 clinic registration fee via Venmo to @douglascountycheerleaders. If Venmo is not a suitable payment option, please contact Diane at 541-680-2202 to arrange an alternative method of payment. Payment must be received no later than September 1st for registration to be valid.
