Paperwork After Delivery
SEND YOUR POD THE DAY YOU DELIVER. YOUR PAYMENT CLOCK DOES NOT START UNTIL WE HAVE IT.

Required on every load:
- PROOF OF DELIVERY (POD) — the delivery receipt or bill of lading signed and dated by the consignee, showing time of delivery, quantity received, and any notation of shortage, damage, or rejection
- Signed origin bill of lading, completed legibly and in full
- Where applicable: lumper receipt, washout receipt, clean truck affidavit, and temperature download or data logger readout
Do not hold paperwork to send several loads at once. If a document is missing, tell dispatch the same day so it can be addressed before billing.
Originals: hold your original paperwork for twelve (12) months. Do not mail originals unless SDI requests them.
Payment terms, methods and fees are set out in Section 8 of this packet.

9. Broker-Carrier Agreement - Signature
This written Agreement, together with the rate confirmations, Exhibits A and B, and the Insurance Requirements, Carrier Safety & Compliance Certification, Carrier Profile Form and Payment Options & ACH Authorization submitted by Carrier with the carrier packet, contains the entire agreement between the parties and may be modified only by a signed writing.
14. Term and Termination - This Agreement is for a period of one (1) year and renews automatically for successive one-year terms unless cancelled. Either party may terminate this Agreement upon fifteen (15) days written notice. Sections 1.1, 1.2, 1.4, 1.7, 3, 3.1, 7, 8, 8.1, 10, 10.1, 10.2, 12 and 13 survive termination.
By signing below, Carrier represents that it has the authority and ability to enter into legally binding contracts and agrees to be bound by the terms and conditions of this Agreement effective immediately.
