This form is to be completed by an MSU Faculty or Staff member to support an undergraduate student's scholarship or award application.
Thank you for taking the time to support this student's scholarship and/or award application. This form has been designed to streamline the reference process and serves in place of a traditional letter of recommendation.

The questions that follow provide an opportunity to share your perspective on the student's engagement, achievements, impact, and goals. We encourage you to draw upon your experiences working with the student and provide specific examples whenever possible. Your insights will help reviewers better understand the student's contributions to the MSU community, their personal and professional growth, and how a scholarship or award may support their future success.

Not all questions may be applicable to every reference. Please respond to those for which you can provide relevant insight based on your interactions with the student.
